SUBJECT: 2014 Cooling Tower Registration & Update Instructions
May 1, 2014 is the annual deadline to update your Cooling Tower Registration
Information with the City of Hamilton Public Health Services. Please visit
www.hamilton.ca/legionella to review and obtain Cooling Tower Registration Forms and
Risk Management Plan Templates.

In 2012 an outbreak of Legionella infections linked to cooling towers in Quebec City
caused 180 people to become ill, of which 13 people died.

In September 2005 an outbreak of Legionella infections linked to a cooling tower in
Toronto caused 135 people to become ill, of which 23 people died. In February 2014
the cooling tower owner agreed to accept responsibility for being negligent in the
design and maintenance of the cooling system and will pay a $1.2 million class action
settlement, plus the class action legal fees of $356,500 and $250,000 to settle past
and future medical expenses for class members.
Instructions to update registration information by May 1st 2014;
1. Obtain blank Registration Forms at www.hamilton.ca/legionella.
2. Complete the Registration Forms. Use your previous registration forms as a
reference.
3. Sign and date Acknowledgement Section
4. Return your updated Registration Forms to:

Cooling Tower Registry By-law 11-078 requires that the Public Health Services be
notified within 10 business days of any information that has changed from the time of
your last Registration.
