Sparks, OK 74869 Cost $150.00 per student Monday 12pm - Friday 11am oym • REGISTRATION th o Registration opens Friday, January 24 . o Turn in your Youth Camp Church Registration form as soon as possible with $40 deposit for each student because camps fill up fast on a first come first serve basis. nd nd o A GUARANTEED MINIMUM number of students and leaders is due by May 2 . Additional students may be added after May 2 for a $10.00 late fee. (Please understand that you will be responsible for the full registration cost for ALL reserved spots after the May nd 2 deadline, even if they cancel at the last minute). • O k l a h o m a Y o u t h M I n i s t r i e s FORMS o All forms needed to reserve your place in the camp of your choice are enclosed. th nd § Church Registration (Turn in ASAP with deposit) After March 28 no refunds for deposits. After May 2 you are responsible for the full $150 cost of camp per student registered. th th § 2014 Staff Application with $45 fee (Postmarked by Friday, March 28 ) $20 late fee after March 28 . § 2014 Individual Camper Registration and Medical Release Forms (Due upon arrival at camp) § $40 deposit per camper. § $45 per staff. o All forms along with details are posted on our www.oklahomayouthministries.com website under events – Youth Camp Information. • ON-SITE REGISTRATION o Noon-3pm in Cabin 5. o Orientation starts Monday 3pm and camp ends Friday at 11am (Please make pick up and drop off arrangements accordingly). o Bring remaining balance. o Bring Individual Camper Registration and Medical Release forms. o All medication must be in original bottle to turn in with the nurse. o 347489 E0990 Rd. Sparks, OK 74869 § 918-866-2407 • CAMPERS o All individual camper registration and medical release forms must be brought with you and turned in at on-site registration at the camp you are attending. o All individual camper registration and medical release forms must be filled out completely. o All medication MUST BE IN ORIGINAL BOTTLE. o A copy of their insurance card needs to be attached with the form. o No student will be allowed to leave camp! Only in case of emergencies and they must be signed out by the parent or guardian on the camper form. Please have parent or guardian bring ID to the office in this case. • STAFF INFORMATION th o All staff members must have a current 2014 staff application completed and postmarked by Friday, March 28 with $45 fee. (An exception may be made as a very last resort and must have the recommendation of their Sr. Pastor, Youth Pastor as well as clearance from our District Youth Office.) th Any staff form postmarked after Friday, March 28 should understand that they may not be accepted. • REFUNDS th o If a change or cancellation is made with your camp registration before March 28 you will be able to use a percentage of that deposit to transfer to another staff member or put towards your overall balance. $20 will be nonrefundable at all times. th o No refunds for deposits after March 28 for students. th o No refunds or transferring one Leaders fee to another after March 28 for Leaders. nd o You will be responsible for the full registration cost for ALL reserved spots after May 2 ($150 per student spot reserved and $45 per staff spot reserved). No refunds will be issued after this point.
