POSITION TITLE: Youth Transition Program Transition Specialist – Mohawk High School
(Grant Funded Position)
CLASSIFICATION: Instructional Assistant
STARTING DATE: ASAP and after required background checks are completed.
WORK YEAR: 20 hours per week (Grant Funded)
BASE RATE: $12.46-13.90/hour per classified collective bargaining agreement
HOURS: Hours will vary, but most hours are determined by high school schedule

Position Description
Mohawk High School is looking for an outstanding individual to work as the Transition Specialist under the
Youth Transition Program (YTP). The Youth Transition Program is funded under a grant from Oregon Office
of Vocational Rehabilitation.
The person in this position is under the general supervision of the Principal and works directly under the
instructor providing vocational support under federal IDEA programs while coordinating with outside agencies.
The YTP Transition Specialist must be adept at balancing school based opportunities for students while
providing a link to external opportunities for students under programs of the Oregon Office of Vocational
Rehabilitation.
Qualifications
1. Meets Highly Qualified standards as defined under federal programs under IDEA and NCLB;
2. Strongest candidates will have ability to organize YTP services for identified students and coordinate
with the school IDEA instructor, Oregon Office of Vocational Rehabilitation Services and school staff;
3. Previous vocational rehabilitation work or similar work in social services is preferred, but a
combination of education and related skills may be substituted;
4. Strong verbal and written communication skills;
5. Applicants should be prepared to document skills in the areas related to working with students with
disabilities and related life skills support;
6. Demonstrated positive behavior management skills;
7. Demonstrated ability to effectively use technology for organization and communication;
8. Ability to systematically and interdependently work with school staff to support development of
vocational opportunities and planning for identified students;
9. Ability to respond to direction and implementation of student support under individualized educational
plans while coordinating with outside agencies;
10. Ability to communicate effectively and work cooperatively and collaboratively with colleagues, support
staff, administrators, students, parents, and patrons;
11. Ability to resolve conflicts and demonstrate flexibility and creativity in solving problems;
12. Must be able to meet all background checks as required under state law.
