SUBJECT: Resolution approving execution of Amendment #1 to the Renewal Contract with Nancy M.
Callahan, Ph.D. effective January 1, 2014, to 1) reflect an official name change to Nancy M. Callahan,

increase the maximum contract price from $117,600 to $197,600(an increase of $80,000)for F/Y
2013/14.
RECOMMENDATION: Approve the attached Resolution.
FUNDING: Contract services are funded by Medi-Cal QA (Quality Assurance) funds, MHSA
(Mental Health Services Act) dollars, Health Resources and Services Administration(HRSA)Rural
Health Outreach Grant funds, and grant funds awarded Behavioral Health for the Blue Shield of
California Foundation Safety Net Integration Project. This Amendment increase will be funded by
Senate Bi1182, known as the Investment in Mental Health Wellness Act of 2013, for both the Mental
Health Triage Personnel and Crisis Stabilization Grant Projects, and Medi-Cal Federal dollars.
Services are within the department's budget. There are no county general fund dollars required in
the Agreement.
BACKGROUND:Per Resolution No. 13-258, the Board approved this year's renewal contract with
Nancy Callahan, Ph.D. for the continuation of consultation services for Quality Management(QM),
Quality Improvement(QI), and special grant development projects.
The Contractor develops Integrated Health Care Grant Proposals on behalf of the County. Integrated
Health Care refers to providing mental health, substance treatment, and physical health services in a

highly coordinated, seamless, and integrated manner. Integration of health care services improves
services to clients, reduces costs, and improves outcomes such as reduced hospitalization, reduction
in need for crisis services, improved health outcomes (e.g. management of diabetes, depression,
weight, etc.), and increased functioning in daily activities.
The Contractor provides consultation services related to quality assurance, Medi-Cal billing policies
and activities, grant development and implementation services. This includes facilitation and
evaluation activities related to the Health Resources and Services Administration(HRSA)Rural
Health Care Services Outreach Grant for the Healthy Outcomes Integration Team(HOIT)Project
and for the Blue Shield of California Foundation Safety Net Integration Project Grant for the
County's Behavioral Health Department.
Most recently, this Contractor provided grant writing activities for two successfully funded SB 82
grants. Pursuant to this amendment,the Contractor is responsible for planning, developing,
implementing and evaluating activities ofthe newly awarded SB 82 Mental Health Triage Personnel
Grant(approximately 2.5 million) and the Crisis Stabilization Grant($500,000). Also, per this
Amendment,the Contractor will provide additional quality assurance activities and technical
assistance, such as preparation for the biannual State Department of Health Care Services Medi-Cal
Audit in June 2014. The contractor will conduct the evaluation components ofthe grants and
develop annual and other periodic reports as required.

services related to quality assurance, Medi-Cal billing policies and activities, grant development
and implementation services for the County's Behavioral Health Department. This also includes
facilitation and evaluation activities related to: the Health Resources and Services Administration
(HRSA)Rural Health Care Services Outreach Grant for the Healthy Outcomes Integration Team
(HOIT) Project; the Blue Shield of California Foundation Safety Net Integration Project Grant;
and the Senate Bill 82 Mental Health Triage Personnel Grant and Crisis Stabilization Grant.
Proiect Overview:
Contractor will support Behavioral Health in enhancing and implementing a comprehensive
Quality Assurance Program, which will utilize data to reduce higher levels of placement (e.g.,
Inpatient, IMD, Board and Care, and Group Home) for clients, develop community client
supports, improve service utilization, maximise Medi-Cal billing, and develop a comprehensive,
coordinated, culturally sensitive integrated service delivery system to promote health and
recovery.
Contractor will also oversee and facilitate grant application processes for any newly released
grant opportunity from available funding sites, upon the request of the Behavioral Health
Director.
Contractor will assist the County with preparation for the annual on-site review conducted by
APS Healthcare. APS Healthcare contracts with the State of California as the External Quality
Review Organization. Contractor will also assist with site visits and audits from the Department
of Health Care Services(DHCS)and the Alcohol and Drug Department.
Contractor will facilitate and provide evaluation activities related to the Rural Health Care
Outreach Grant: Healthy Outcomes Integration Team Project as approved per Resolution No.
12-158, and the Blue Shield of California Foundation Safety Net Integration Project grant.
Contractor's responsibilities shall include but not necessarily be limited to the following:
A. On behalf of Nevada County Behavioral Health, Contractor agrees to provide the following
Quality Management (QM), Quality Improvement (QI), and Special Projects assistance for
outpatient services:
1) Assess County status and needs in regards to the development and implementation of a
DHCS required Cultural Competency Plan("CCP").
2) Upon County request, organize and conduct meetings of the Cultural Competency
Committee necessary in order to develop and approve a CCP.
3) Upon County request, conduct training activities in consultation with COiINTY staff on
the CCP.
4) Assist the County with preparation for annual on-site reviews conducted by APS
Healthcare. APS Healthcare contracts with the State of California as the External Quality
Review Organization. The External Quality Review(EQR)is an annual audit conducted
by an independent review organization.

Activities in preparation for the EQR includes reviewing policies and procedures;
developing data to analyze timeliness of access to services, including medication
services, analyzing no show data, and follow-up services after a hospitalization;
developing two Performance Improvement Projects; discussing evidence based practices;
and developing data for measuring client outcomes.
5) Train and provide support to the Behavioral Health Quality Assurance Manager to
implement an exemplary Quality Improvement System, including Utilization Review,
Medi-Cal Billing and documentation, timeliness of services, Site Certifications,
Credentialing, developing the Annual Work Plan, Performance Improvement Projects,
Managing High Service Utilization of Inpatient, IMD, Group Home, and Emergency
Services.
6) Support Behavioral Health and Organizational Providers to accurately submit claims for
Drug Medi-Cal.
7) Develop management decision support data for managing data on Inpatient and other
high cost services on a monthly basis (children and adults). Develop strategies to reduce
inpatient hospitalizations and resolve crises in the community, when possible. Provide
data on group home and out-of-home placement to help county manage costs of
placement.
8) Provide support to the Behavioral Health Quality Assurance Manager in developing
policies and procedures to ensure compliance with all state and federal quality assurance,
Medicaid and Medicare requirements, including Katie A., integrated health care, etc.

10) Assist with Provider Certifications.
11)Incorporate Nevada County into Contractor's existing multi-county contract with
Essential Learning for online staff training. This training program provides aweb-based
site for the Nevada County Behavioral Health Workforce, and included clinical and CEU
(Continuing Education Units) courses related to mental health and substance abuse
treatment.
12)Perform other activities related to Quality Management as needed and requested,
including training staff on documentation, HIPAA, compliance, and development and
implementation of the Performance Improvement Projects, implementation of the
Affordable Care Act, and review and development of policies and procedures to comply
with state and federal regulations.
B. Integrated Health Care and/or other Behavioral Health Department related grants:
Project Overview:
Integrated Health Care refers to providing mental health, substance use, and physical health
services in a highly coordinated, seamless, and integrated manner. Integration of Health Care
services will greatly improve services to clients, reduce costs, and improve client outcomes.
These outcomes may include reduced hospitalizations, reduced crisis services, improved health
outcomes (e.g., cholesterol, diabetes managzment, weight, de~,ressionj, and iticieased
functioning in daily activities.
• Contractor shall coordinate efforts of the California Institute of Mental Health Care
Integration Collaborative (CIC), facilitate meetings, and develop documents required as a
component ofthis collaborative.

• Contractor shall be responsible for developing, implementing, and evaluating
successfully awarded grant activities related to integrated healthcare.
Contractor shall be responsible for developing other grant proposals, as directed by County,
during the term of this contract.

for the Rural Health Care Outreach Grant: Healthy Outcomes Integration Team (HOIT). The
Contractor shall also be responsible for conducting the evaluation components of the grant, as
outlined by HRSA and developing the annual, and other periodic reports, as required by HRSA.
Contract duties shall include but not necessarily be limited to:
• Developing, designing, and implementing evaluation activities for the project. This
includes collecting data on access, quality, cost-effectiveness, and positive health
outcomes.
• Maintaining collaboration with HOIT agencies regarding Management Information to
more effectively collect performance outcome data for national evaluation.
• Contractor shall provide professional consultation regarding analyzing, interpreting, and
reporting outcome results.
• Contractor shall prepare periodic reports and present information to key stakeholders
group.
• Contractor shall facilitate grant meetings with stakeholders and coordinate activities and
provide documentation to ensure compliance with all federal funding requirements.

Evaluation Activities and Periodic Reports
Contractor shall conduct evaluation activities, including providing support and leadership to all
evaluation components of the Triage Personnel Grant and the Crisis Stabilization grant. This
support includes facilitating the evaluation team to design all components of the evaluation
activities, including identification of key outcomes; collection of data at all key points of service
in the crisis continuum of care; and ongoing evaluation of data to assess access, quality, costeffectiveness, and outcomes.

b) Employment details of all triage personnel. This information will include the date of
hire, level of education, level of experience (including lived experience), race/ethnicity,
languages) spoken, and cultural strengths. This information will be collected and
reported by both county and contract providers.
c) Information on each crisis service delivered. This data will include the date of service,
the length of service, type of service, location of the service, and the outcome of the
service. When the individual is referred to another agency, this information will also be
reported.
d) Information on each client will include a unique identification for each person served,
basic demographic information(ED, psychiatric hospital, SNMH,Insight Respite Center,
crisis stabilization program, homeless shelter, jail, NCBH clinics, FQHC clinic, contract
providers, and other community locations).
Evaluator will analyze the data and provide reports to management and stakeholders on a routine
basis. This information will include:
a) Total unduplicated persons served (by service provider and total).
b) Total number of service contacts(by type of service, service provider, and total).
c) Basic demographic information for each person served including age, race/ethnicity,
preferred language, gender, cultural heritage, LGBTQ2-2S, and military status.
d) Linkage and referral information including where each individual was referred by crisis
triage staff.
e) Information regarding whether each individual linked to a NCBH mental health service
received follow-up care. Information for the NCBH Anasazi system will be used to show
if the individual received a mental health service, the type of service received, and length
of time between the most recent mental health service and the date of the triage service.
A summary of data from this encounter-based information will be reported at 12 months,
and every 6 months, thereafter, through the end of the grant cycle. This evaluation data
will also be used to provide ongoing feedback on access, quality, and outcomes.
2. Program Effectiveness
Evaluation data will be collected, analyzed, and shared with providers, county staff, and
stakeholders to continually assess the crisis continuum of care to ensure timely access, quality of
services, and system and individual outcomes. Frequent review of this information will
determine the effectiveness of services. This information will help identify barriers to timely
access and quality care, and help revise services to meet the needs of the individuals served. The
crisis continuum of care, which includes providers, peer counselors, service recipients, family
members, and interested stakeholders, will review data at least monthly within a Quality
Improvement framework. As the group reviews data, we can assess the quality of services,
identify areas for strengthening care, and celebrate positive outcomes.

Evaluation activities include developing and writing required reports at semi-annual and annual
intervals. These reports will be in response to required grant funding reports as well as to inform
stakeholders and community members. In addition, evaluation reports will be available to the
crisis contin~~2~m of care team to continually improve access, qualit3~, and outcomes.
F. Crisis Stabilization Grant(SB 82)
Evaluation activities for the Crisis Stabilization grant include analyzing the average time to
access services, including the time the individual enters the ED, or presents in the community
needing crisis support services, the time a mental health crisis worker greets the individual, the
length of time to triage and determine the individual's needs, and the length of time to linkage to
the appropriate service, including admission to CSU, linkage to the Insight Respite Center,
admission to an inpatient psychiatric hospital, and/or discharge and return to their family and
local community.
• Hospital emergency room and psychiatric inpatient utilization.
• Reduced law enforcement involvement on mental health crisis calls, contacts, custodies
and/or transports for assessment.
• Consumers' and/or their family members' satisfaction with the crisis services the
consumer received.
• Utilization of Crisis Stabilization beds.
• Description of persons served.
• Cost effectiveness of services.
• Recidivism of persons served by Crisis Stabilization Program.
Data will be collected at the point of contact with the client, entered into Anasazi and/or
completed on paper or electronically. All contract providers in the crisis continuum of care will
be required to report information to Nevada County Behavioral Health and the evaluation team
as a component of the grant.
Evaluation activities include collecting and analyzing data and producing reports required by the
finding agencies, in collaboration with NCBH Fiscal Services.

Proposed budget in accordance with the provision of services in Exhibit"A" shall allow for:
MHSA,Integrated Health Care Project, CiMH Care Integration Collaborative (CIC): $41,600
Quality Assurance Activities and Technical Assistance: $47,079
Health Resources Services Administration Grant, Healthy Outcomes Integration Team (HOIT)
Implementation and Project Evaluation: $40,000
Blue Shield of California Foundation Safety Net Integration Project, facilitation and evaluation
services: $15,000
Essential Learning- online staff training: $6,000
SB 82 Grants — Planning, implementation, evaluation, and reports: $47,921
Expenses for travel necessary for performance of contract services may be reimbursed up to a
$6,000 cap, and the total paid to Contractor including travel expenses shall not exceed the
contract maximum.
Mileage reimbursement shall not exceed the current IRS allowable reimbursement rate.
Contractor shall provide on invoices a breakdown listing specific services provided, the dates)
and numbers) of hours, and any itemized travel related expenses that are being billed.
Contractor shall submit receipts for any lodging, meals, and and/or car rental/mileage for
reimbursement by the County.
BILLING AND PAYMENT:
Contractor shall submit to County, for services rendered in the prior month, and in accordance
with the reimbursement terms, a statement of services rendered to County and costs incurred that
includes documentation to support all expenses claimed by the 20th of each month. County shall
review the billing and notify the Contractor within fifteen (15) working days if an individual
item or group of costs is being questioned. Contractor has the option of delaying the entire claim
pending resolution of the cost(s). Payments of approved billing shall be made within thirty (30)
days of receipt of a complete, correct, and approved billing.
