Dear OUMC Affiliates:
Healthcare facilities, including OU Medical Center, are taking steps to ensure we do everything we can to
protect our patients, our families and our co-workers from the seasonal flu.
At OU Medical System, we expect every individual having patient contact to be vaccinated for the flu. Any
employees, students, contract workers and other individuals having patient contact who decline the
seasonal flu vaccination will be required to wear a surgical mask for their entire shift beginning November
3, 2014, in all patient care, procedural and registration/admitting areas. We currently anticipate this
requirement ending on March 31, 2015, but the flu season is different every year and we will adjust this
date should this flu season extend beyond March.
We ask that you notify your workers or students assigned to an OU Medical System campus of our
requirements. If you do not offer flu vaccinations, please advise them that the vaccination is available to
them at the Oklahoma State Health Department, County Health Departments, local pharmacies, many
primary care physicians, and other sources.
To be compliant with HCA and Joint Commission tracking and reporting requirements, all facilities,
organizations, and schools must provide OU Medical System with an attestation regarding vaccination or
declination for each contracted individual or student who will be in our facility between November 3, 2014
and March 31, 2015. A direction page with the spreadsheet template and an attestation form is attached.
Badge stickers indicating vaccination will be issued by the OU Medical Center Human Resources Office
in the Professional Office Building at 711 Stanton L. Young Blvd, Suite 103 to the school
coordinators/instructors and company contacts with the understanding that the stickers will be given only
to individuals that have received their vaccinations and provided the necessary documentation. Workers
and students without the badge sticker will be expected to wear a surgical mask as outlined above.
Please sign and return the acknowledgment and confirmation form concerning receipt of this information
and verifying that our requirements have been communicated with your employees or students. You may
email or fax the form back to Chris Patrick. We greatly appreciate you r cooperation and participation in this
vital public health effort.

practices will make a significant difference in our facilities and our communities. If you have any
