YELLOW FEVER VACCINE: FOR all the individual travelling for East African Countries (KENYA, TANZANIA, Uganda, Rwanda, Burundi): Patients' travelling from East African countries requires taking yellow fever vaccine before 15 days of travelling. ORAL POLIO VACCINE •
The Government of India has decided that all travelers to India, children as well as adults, from republic of Kenya will need to have one dose of oral Polio Vaccine(OPV) at least six weeks prior to entry into India. •
Accordingly, Government of India has decided to make it a mandatory requirement for all travelers from WHO‐identified seven countries including Nigeria, visiting India to have OPD six weeks before departure for India. The OPV is valid for one year from the date of its administration. This mandatory requirement will take effect from 30th January 2014. The OPV is also a mandatory requirement for all Indian Nationals visiting Nigeria and the remaining six WHO‐identified countries i.e. Afghanistan, Ethiopia, Israel, Kenya, Somalia and Pakistan. •
Travellers of all age originating from any of the above WHO‐identified seven countries may kindly ensure that they get OPV administered through a Government approved medical centre/clinic/Institute/Hospital ( Like MP shah and Agakhan hospital)six weeks prior to their travel to India and carry along the polio certificate. This is a mandatory requirement with effect from 30.01.2014. DOCUMENTS REQUIREMENT APPLICATION PROCESS: 9
Visa applications ARE ONLINE APPLICATION ONLY. You will require to log on to Indian embassy website and fill the form online. Following that, You will require to submit Filled‐in and signed hard copy of the online visa application form with following documents: 9
Two recent colour photographs. 9

Medical documents establishing the need for taking medical treatment in India. This means medical report from your doctor OR a referral letter from local doctor Or hospital. 9
Proof of availability of sufficient funds for stay in India including the expenses for medical treatment by producing documents such as Bank statement, Salary certificate, sponsorship letter etc. 9
Fee: Ksh.7600 + Ksh 200 towards ICWF for six months & Ksh.11400 + Ksh 200 towards ICWF for one year. 9
A Medical/Medical Attendant visa is normally issued in 3 working days, exclusive of the day on which application is made. PROCESSING TIME •
All applicants are required to come personally for biometric data capture. However, following are exempted from personal appearances:‐ •
Diplomatic/ Official Passport holders. •
Applicants below the age of seven years and above the age of 70 years. •
Medically ill applicants who are unable to come physically. •
Visa to Tanzanian nationals is normally issued in three working days, and for the residents of other countries in five working days. •
Applications could be submitted on any working day between 0900 ‐ 1030 hrs. Please see list of holidays for the year 2014. VISA LETTERS •
Patients are requested to take MEDICAL VISA whenever they are travelling for treatment. You will require two letters for visa process. One letter from local doctor stating that you require travelling to India for medical treatment. Another letter will be provided to you from hospital in India. This letter will be directly sent to High commission of India. The spine clinic will assist you by providing these required letters. You need to provide scanned copies of patient's and accompanying person's passport to The Spine Clinic.
