Completing the My Health Declarations form
This information sheet will help you to use the Department of Immigration and Border
Protection’s My Health Declarations service.

What should I do first?
Read the advice regarding when to use this service on the My Health Declarations support
page at:
www.immi.gov.au/allforms/health-requirements/my-health-declarations
When you are ready, click on the link from the support page called Start My Health
Declarations.
Login to your ImmiAccount
You will then be directed to the department’s online account (i.e. “ImmiAccount”) home page.
Here you can:

login to your ImmiAccount if you already have one, or
create a new ImmiAccount to access the department’s online services if you don’t
already one. Just click on register a new ImmiAccount at the bottom of the page, and
follow the instructions provided.

I have logged into my ImmiAccount. What do I do now?
The My Applications Summary page will appear. Click on the New Application button.
Click on the My Health Declarations link in the application list provided.

The My Health Declarations application form will open for you to complete.
Terms and conditions
The first step is to view the terms and conditions and the privacy statement using the links
provided. You need to agree to these before clicking on the ‘next’ button.
Visa subclass details
Select the visa subclass that you intend to apply for from the list. This is mandatory,
otherwise we cannot determine which health examinations (if any) you are required to
complete.

Personal details
On pages three and four of your My Health Declarations form, fill out your personal details,
including passport and previous travel information, using the fields provided.
If you are confused about the question, click on the question mark logo for more
information.
If you fail to complete a mandatory field, a system error will display on the screen. You
must provide this information before continuing.
Additional identity questions
On the next page of your My Health Declarations form, you need to record additional
information if you have previously travelled to Australia or previously applied for a visa.
Note: If you have previously travelled to Australia or previously applied for a visa, make
sure you have the following information ready as you will need to enter it here:
• details of any expired passports that you previously used to travel to Australia
• details of any visa grant numbers for any Australian visas that you have previously
been granted, and
• details of the visa label numbers (where applicable) for any Australian visas that
you have previously been granted.
Contact details
On the next page of your My Health Declarations form, fill out your contact details.
Family members included in your application
On the next page of your My Health Declarations form, indicate whether you wish to include
any other family members in this application. If you say yes, you will be able to determine if
any of your family members will also be required to complete health examinations using the
one My Health Declarations form.
Note: Any family members who do not have a current valid passport will not be able to be

included in your My Health Declarations form.
Note: If you intend to apply for a permanent visa application, any close family members who
are not migrating to Australia may also require immigration health examinations in order for
your visa application to be finalised.

You can include family members in your My Health Declarations form if you would like
them to complete their health examinations ‘upfront’. Your case officer may not request
health examinations for non-migrating family members where it is considered
unreasonable to do so. Consequently, you may wish to wait until after you have lodged
your visa application before completing health examinations for your non-migrating family
members.
Travel details
On the next page of your My Health Declarations form, provide information regarding any
previous travel to Australia, and the details of your proposed stay in Australia. Please
make sure you provide accurate information to ensure the correct health examinations
are generated for you and any family members, where required.

If you are applying for a temporary visa application, you will be asked two additional
questions regarding whether you intend to apply for a permanent stay in Australia in the
next six to 12 months, and if so, whether you wish to have your health assessed for a
permanent stay in Australia.
If you answer yes to these questions you will be asked to undergo full permanent health
examinations (subject to your age), that is a medical examination (501), an x-ray
examination (502) (if aged 11 or over) and an HIV test (707) (if aged 15 or over).
You may wish to undergo these examinations and be assessed for permanent stay in
Australia if you intend to apply for a permanent visa in the next six to 12 months. For
example, some clients may wish to complete permanent health examinations ‘upfront’
particularly if they are applying for a temporary work visa, and intend to apply for
permanent skilled migration once in Australia.
When deciding whether or not to say ‘yes’, you should take into account:

what visa(s) you are applying / intend to apply for and the average processing times
most health examinations are only valid for a 12 month period
some health clearances are only valid for six months (for example where a ‘health
undertaking’ is requested)
your case officer may still ask you to complete additional health examinations for a
subsequent visa application if, for example, your circumstances have changed.
Note: You will need to complete these additional tests at your own cost. If you are not
prepared to pay additional fees and undergo the additional tests, do not select Yes or it
may delay the processing of your visa application.
Health declarations
On the next page of your My Health Declarations form, answer the health declaration
questions that appear on the screen. These questions are the same questions that will
appear in the health section of visa application forms (when you lodge it at a later time).
You need to complete these ‘upfront’ for the department to determine what health

examinations you are required to do (if any).
Note: please be careful when recording the answers to these questions. If you make a
mistake, this may delay the processing of your health examinations and you may be asked
to reschedule your appointment with Medibank Health Solutions or a panel clinic.
Note: When you lodge your visa application, you will again be asked to complete these
questions as your circumstances may have changed in the period between completing your
My Health Declarations form and lodging your visa application. Please make sure you provide
accurate and up-to-date information to avoid any processing delays.
If you answer ‘yes’ to any of the questions on this page, an additional window will open
asking you to select which person on your form the ‘yes’ answer applies to, and to provide
additional details. Please provide the additional information requested and then click on
‘confirm’.
You will return to your My Health Declarations form, where you will see the information you
have entered within a table, see example below.
Complete declarations
You are almost done. Complete the important declarations on the Declarations page to
confirm that you understand the importance of providing true and correct information on
this form, and that you hav e read the available information about the My Health
Declarations service.

Review your answers
All the information that you have entered in your My Health Declarations form will appear on
the next page for you to review.
Please check that all the information you have provided is correct before proceeding, as
once you proceed past this screen you will be unable to change your answers and the form
will be submitted.
Submit your form
Once you click on the Next button from the review page, your form will be submitted.
