Newsletter
Hello and welcome to the fourth edition of Newport’s Neighbourhood Care Network
(NCN) Newsletter, which aims to keep you up to date with what is happening in the
NCNs in Newport.
These Newsletters will share with you some of the topics discussed at our NCN
meetings whilst introducing you to what services are available in our locality. They
will also to provide you with a means of sharing any information you would like to
let colleagues and partners know about.
Developing Cluster Network Action Plans in Newport
Lots of activity is taking place across all NCNs in Gwent to set up the support framework which will help NCNs to develop their Cluster Network Plans, in line with GMS
Contract and QAF requirements.
Practice Development Plans (PDPs): The first stage of the process is for each
local GP practice to populate their PDP with details of the services that their practice
provides and issues raised from their review of local need.
PDPs have to be submitted to the NCN Leads by 30 June but if plans can be with the
Leads by 15 June this would be gratefully appreciated as it gives them a small
amount of additional time to pull all of the PDPs into an NCN Cluster Plan.
Cluster Network Action Plan: The PDPs will be used to inform the development of
a Cluster Network Action Plan. GPs will be then asked to participate in a series of
four meetings to discuss with peers and partners the health needs and service development priorities for the population served by their NCN area.
These issues will be written into a Cluster Network Action Plan.
The Plan will include: a map of local GP services, priority actions the NCN Cluster
want to take forward regarding access arrangements, integration between health
and social care services; maximizing community resources to meet local needs and
strengthening links with the Third Sector.
Each Cluster Network Action Plan has to be agreed and written by 30 September. In
ABUHB we hope to have these drafted by the end of July to avoid the inevitable difficulties of the school summer holiday break.
A further meeting is being organized for GPs and partners to come together to
agree a Cluster Network Annual Report which needs to be submitted to ABUHB
by end March 2015.
Details of when and where these meetings are taking place will be issued very
shortly.

ASD Info Wales, the national site for Autistic Spectrum
Disorders (ASD).
It is estimated that ASD affects 1 in 100 people. The National
site for the Autistic Spectrum Disorders is aimed at those
working with children and adults with ASD.
It provides information about ASDs including Autism and
Asperger Syndrome, service details and training opportunities,
together with downloadable resources that can be shared with
individuals with ASD, their family and carers.
Just added is detail on the ASD planner phone app that can
now be downloaded on Android or Mac platforms.

Community Voice Programme: Finding out what care home
residents want from their services
The Royal Voluntary Service (RVS) has been awarded Community Voice Programme Big Lottery Funding to set up “Care to Listen … Listen to Care” projects
in Newport, Caerphilly, Blaenau Gwent and Monmouthshire.
The projects will work in partnership with residential care providers, placing volunteers in residential care homes so that they can help identify ways in which the
services could be more tailored to take into account the views and concerns of
older people. They will also work in association with Aneurin Bevan’s Care Home
Ask and Talk (CHAat) initiative which was set up to find out how nursing home
residents and their families view the care they receive.
Nina Finnigan, Residential Care Service Manager explains:”Our service will match
the older person with a volunteer who can talk to them, build up a strong rapport
and find out their views about their care”.
“We hope to very much support the care providers, adding value to what they offer and sharing good practice. Staff from the NHS and Local Authorities have
been involved in the development of the project and will continue to be involved
throughout the project”.
“The Care home we are starting with in Newport is Glenmore Residential Care
Home on Stow Hill”.
Nina added: “The Project is looking for volunteers, ideally with a background in
health or social care or working with older people. Volunteers will all receive
training and support and together we can develop and share good practice to ensure that older people have the best possible quality of life.

Introducing the Carers Team
The Carers Measure Team are a small
team of staff based in Llanarth House
Newbridge. Their role is to ensure the
early identification of Carers; provide
them with effective, relevant, up to date
and targeted information and to inform
them of their right to an independent assessment of their needs.
Their achievements so far.......
They have trained over 700 health, social care and voluntary sector staff,
from chief executives to operational
staff, raising awareness of Young and
Adult Carers issues.
They have started identifying Carers
Champions across the health board,
GPs, Local Authorities and the voluntary sector
A Carer aware e-learning package has
been developed which is available for
all partners. A Young Carer specific elearning tool is currently under development.
They have produced separate information posters for Young and Adult
Carers which are being distributed
across health, social care and voluntary sector settings including all
hospital and primary care settings.
They have developed a team intranet page for all Health Board staff
to access, which gives information
about the Carers Measure and how
to access training and a Young
Carers internet web page is under
development following a consultation with Young Carers regarding
content and design.
A GP Local Enhanced Service (LES)
has been developed in recognition
of the central role played by GP’s in
identifying and supporting Carers.
To date 56 out of 89 General Practices in the ABUHB area have
signed up to this service

Telecare Launch 20 May, Newport Centre
Newport City Council Social Services/Supporting People are launching details of
their new Telecare Providers/equipment at the Newport Centre on 20 May.

National Exercise Referral Scheme (NERS)
The National Exercise Referral Scheme’s referral dispensing system is available
to all Health Professionals. This allows easy access to relevant Referral Forms
and Patient Information leaflets. To access the form go to the
link www.nersdb.info 1) Select your county I.E. NEWPORT, 2) Type in the
password webucate, 3) Click next.
In addition, there is also a NERS Health Professional Leaflet and a link to
Motivate 2Move which can be found on the website’s introductory page.
