Game nights: (single games during school and double headers during summer break)
O 8U – Wednesdays
O 10U – Mondays
O 12U – Tuesdays
O 14U – Thursdays
O High School – Wednesdays
Practices: Will begin around mid-May, weather permitting or at coach’s discretion
Fee: $80 (due March 22, 2014)
End-of-Year Tournament: July 18-20, 2014, Watertown

Fees
Fees include Amateur Softball Association (ASA) insurance, ASA league fees, field maintenance fees, paid entry for
the Windmill Travel League end-of-year tournament, and tournament t-shirt. Registration deadline is
March 22, 2014. A $15 late fee will be applied if registration is postmarked after March 22, 2014.
Equipment
All players are required to have their own glove. All players must wear a helmet with a face mask per ASA rules.
Helmets are provided but players are encouraged to have their own. Other equipment provided (but players may
bring their own) includes bat, batting glove(s) and mouth guard (encouraged).
All Players Welcome
The CSA welcomes all girls to play, even if they are not Columbus residents or do not attend Columbus Schools.
Team Placement
Player placement will be based on ability. After April 15, 2014, placement is contingent on team availability.

Registration forms may be obtained at all Columbus Schools, the Columbus Recreation Department, the Columbus
Public Library or online at the City of Columbus Recreation webpage
(http://www.cityofcolumbuswi.com/CityDepts/Recreation/rec.asp). (Online registration fee is up to $2.25.)

As a parent or guardian of the aforementioned child, I hereby give my permission for her participation in
softball for the 2014 summer season. I hereby state that I will notify my daughters coach as soon as
possible if I am aware that she will not be able to attend a game. I also state that I will not hold CSA, the
league, coaches, the City of Columbus, or persons transporting my child to and from softball activities
responsible for any accident or injury that may occur. I further state that I will be financially responsible
for the return of and care of any equipment and uniform issued to my daughter.
