INSTRUCTIONS（記入上の注意）
1. The application should be neatly handwritten in BLOCK letters. （明瞭に記入すること。)
2. Each applicant must submit the following complete set of documents along with the application form by 12 December 2015（応
募者は、以下の書類一式として 12 月 12 日までに提出する。）

3. All documents are to be SEALED, with ID, name, and contact details written on the envelope. Personal data on this form will be
used ONLY by the Homestay Committee for homestay selection purposes.
（書類は封筒に入れて封印をして、封筒表面に身元番号、氏名及び電話番号を記入すること。本申請書に記載された個人情報に
ついては、本ホームスティの選考のために委員が使用する以外には使用しない。）
4. A confirmation mail with the address of [email protected] will be sent within 48 hours of receiving the application. （48 時
間以内に確証メールは応募者に送信を行う。）
5. Screening will be performed by means of submitted application documents and interviews.
（申請書類及び面接に基づき、選考を行う。）
6. Results of the selection will be announced by January 2015. （選考の結果通知は 9 月とする。）
7. Successful applicants are required to prepare a sum of RM4, 500 as the total expenditure for the trip.
（選考合格者は、RM4,500 の経費を用意すること。）
