ISBE Indicator 13: Secondary Transition 2014-2015 Evidence of Correction 1. Criteria: Does the IEP include a measurable post-secondary goal for the following? • Education and/or Training • Employment • Independent Living 34 CFR 300.320(b) Corrective Reconvene or amend the IEP within 60 days from the notification of noncompliance. Review policies/procedures to ensure that the district/school will write IEPs that include post-secondary goals that are measurable statements and occur after high school graduation/aging-out. Actions: Is the postsecondary goal addressed/updated annually? 2. Criteria: • Education and/or Training • Employment • Independent Living 34 CFR 300.320(b) 3. Corrective Reconvene or amend the IEP within 60 days from the notification of noncompliance. Review policies/procedures to ensure that the IEP team will address or update each postsecondary goal in conjunction with the current IEP. Actions: Is there evidence that the measurable post-secondary goals were based on age-appropriate transition assessments? Criteria: 4. Corrective Reconvene or amend the IEP within 60 days from the notification of noncompliance. Review policies/procedures to ensure that the district/school will utilize transition assessments that provide information on “the student’s needs, taking into account his/her strengths, preferences and interests” Actions: regarding post-secondary goals. For each post-secondary goal, is there a type of instruction, related service, community experience, or development of Criteria: • Education and/or Training • Employment • Independent Living 34 CFR 300.320(b) employment and other post-school objectives and, if appropriate, acquisition of daily living skill(s) or provision of a functional vocational evaluation listed in association with meeting the post-secondary goal? • Education and/or Training • Employment • Independent Living 34 CFR 300.320(b) Corrective Reconvene or amend the IEP within 60 days from the notification of noncompliance. Review policies/procedures to ensure that the district/school includes transition services that align to each post-secondary goal; focuses on improving the academic and functional achievement of the student to Actions: assist in their movement from school to post-school settings. ISBE Indicator 13: Secondary Transition 2014-2015 Evidence of Correction Does the IEP/transition plan include a course of study that will reasonably enable the student to meet his or her postsecondary goals? 5. Criteria: 6. Corrective Reconvene or amend the IEP within 60 days from the notification of noncompliance. Review policies/procedures to ensure that the district/school includes a course of study that is aligned to the student’s post-secondary goals and covers the remaining years that he/she is in secondary education Actions: Are there annual IEP goals that are related to the student’s transition services needs? Criteria: 7. Corrective Reconvene or amend the IEP within 60 days from the notification of noncompliance. Review policies/procedures to ensure that the district/school includes at least one annual goal and short-term objective in the IEP that is related to the student’s transition needs. Actions: Is there evidence that the student was invited to the IEP team meeting where transition services were discussed? Criteria: 34 CFR 300.320(b) • Education and/or Training • Employment • Independent Living 34 CFR 300.320(2)(i) 34 CFR300.321(b) 8. Corrective Reconvene the IEP within 60 days from the notification of noncompliance. Review policies/ procedures/regulations to ensure that all students aged 14 ½ or older are invited to IEP team meetings where transition services are discussed and the Notice of Conference is signed and dated prior to the IEP Actions: meeting by the LEA representative. If appropriate, is there evidence that a representative of any participating agency was invited to the IEP team meeting with the Criteria: prior consent of the parent or student who has reached the age of majority? . 34 CFR 300.321(b)(3) Corrective Reconvene the IEP within 60 days from the notification of noncompliance. Review policies/procedures to ensure that the district/school will invite a representative of any participating agency providing there is prior consent from the parent/guardian or student who has reached the age of majority. Actions: Does the IEP meet the requirements of Indicator 13? YES NO Results reflect only those IEPs reviewed in this activity and, as such, might not be representative of the entire district. All districts are expected to enter SPP Indicator 13 data for students aged 14½ and up who have an IEP and also comply with the Federal requirement of 100% compliance for each and every student aged 16 and up who has an IEP. As of August 2013, all districts must comply with changes to 105 ILCS 5/14-8.03 which makes Independent Living a State requirement.
