The premiums are exempt from both Federal and FICA taxes. This benefit plan allows employees to pay for health care expenses and/or dependent care expenses with pre-tax dollars. Term life insurance equal to one times annual salary (Up to a maximum of $ 50,000). Good Samaritan Society Good Samaritan Society Accidental Death & Dismemberment coverage (Up to a maximum of $ 50,000). Employee Additional term life coverage (1-5 times salary to a maximum of $350,000).
