2015 Benefit Re-Enrollment Highlights This handout highlights the benefit changes effective January 1, 2015. Additional information is available on the benefit re-enrollment web site. Allow time to review. Workday is your one-stop for the 2015 Benefit Re-Enrollment season! Re-Enrollment is a two-step process. Step 1; review the changes for 2015. Step 2, complete your Workday Open Enrollment action located in your Workday inbox by the deadline of Monday, December 1, 8:00 a.m. Central Time. SECTION 125 (PRE-TAX) PLANS: Marshfield Clinic has partnered with Diversified Benefit Services Inc. (DBS) to help in the processing of reimbursement requests for the Dependent Care, MEFSA and LPFSA plans. Dependent Care Plan: No changes. Premium Conversion Plan: Health and dental insurance premiums will default to pre-tax deductions. Medical Expense Flexible Spending Account (MEFSA): No changes, however, a $500 carry over is allowed beginning with the 2014 plan year. Limited Purpose Flexible Spending Account (LPFSA): No changes, however, a $500 carry over is allowed beginning with the 2014 plan year. Health Savings Account: Statutory maximum contributions increased to $3350 single and $6650 employee +1/family. Pre-tax HSA is available through Fidelity Investments. Post-tax HSA contributions, from your paycheck to a non-Fidelity account, can be entered under Workday’s Pay worklet and Payment Elections. DENTAL INSURANCE: OPEN ENROLLMENT. Elections completed in Workday by the Dec 1 deadline will be effective January 1, 2015. Dental Com Plan: No changes. Delta Dental Plan: 1.7 percent premium decrease and enhanced dental benefits for those who need them most. See Dental Benefits Comparison for additional details. HEALTH INSURANCE Marshfield Clinic’s HMO plans will be replaced with Point of Service (POS) options. Participants will continue to have indemnity plan options. Active Advantage POS and indemnity plans will cover two problem-focused office visits per year with a Primary Care Provider (PCP). 2015 Active Advantage health plan participants will need to submit the name of their PCP to SHP prior to receiving care. Maintenance medications must be filled at a Marshfield Clinic pharmacy. New (first month) and one-time prescriptions may be filled at a non-Marshfield Clinic pharmacy once per year. Tier 1, generic prescription drug copayment will change from $3 to $5 per 1-month supply. A new High Deductible Health Plan option will be added. Each health plan will have a POS and indemnity option resulting in a choice between six health plans. Premium increase, based on utilization, is 6% (total monthly premium). The Clinic’s monthly contribution for a full-time participant will change from $1226.50 family, $1066.52 employee+1 and $533.27 single to $1150 family, $1000 employee+1 and $500 single.
