JJC FAMILY SERVICES JOB ANNOUNCEMENT JOB TITLE: Case Manager for Face Forward 2 Workforce Development Program LOCATION: 619 N 35th Street Phila. Pa. 19104 ROLE: TheFaceForwardIICaseManagerwilladdressthemultiplebarriersto employmentfacedbycourtinvolvedandatriskyouthbetweentheagesof14and 24livingintheWestPhiladelphiaarea,primarilythePromiseZone.Thecase managerwillbepartofateamwhosemissionistocoach,teach,andinspireeligible participantstodeveloptheskillstheyneedtobecomeemployableandindependent andplacingtheminmeaningfuljobsandorpostsecondaryeducation or vocational training in industry recognized certification programs. Collaboration with the family, school, juvenile justice officials, community based organizations and other supports is a mandated feature of this position. Case manager will be responsible for job coaching, building relationships with community stakeholders, public speaking and life skills instruction. STATUS: This position is a full time position. SALARY $30,000.00 to $32,000.00. HOURS OF WORK: Assignments are based on a 40-hour week. Weekly schedules are developed based upon the on- and off-site programming and job readiness and workforce development projects. Agency accountability rules apply, so that the program director knows where workers are at all times when they are at work. There are no traditional office hours. Case managers will do case management primarily in the afternoon and evenings, specifically during the school year and at weekends to provide maximum involvement with the identified clients and family. Case managers will be placed on a roster to ensure office coverage. The case manager will be placed on an on-call roster and will be responsible for providing emergency assistance to clients assigned to the program 24 hours a day while on duty. ESSENTIAL REQUIREMENTS A bachelor’s degree in human services, preferably social work (BSW) counseling, Education, or related degree. Four years experience working with at-risk or court involved youth required. Excellent written and verbal communication skills and computer proficiency also required. Valid driver’s license with an acceptable driving record that will result in JJC’s insurance carrier’s approval. An annual physical form verifying that the employee is “free from contagion". Satisfactory PA Child Abuse, PA Criminal, and FBI Clearances. Copy of official transcripts needed at time of interview to review all relevant coursework. The ability and willingness to adjust work hours in accord with program hours and clients’ schedules. The ability to meet all deadlines and participate in supervision and mandated training.
