φακζλου υγείασ για κάκε πολίτθ που είναι εγγεγραμμζνοσ ςτον κατάλογό
του, διαςφαλίηοντασ τθ ςυνζχεια και το ςυντονιςμό τθσ φροντίδασ,
κακϊσ και τθν αποτελεςματικότθτά τθσ. Με απόφαςη του Υπουργοφ
Υγείασ κακορίηονται οι λεπτομζρειεσ τθσ εφαρμογισ του θλεκτρονικοφ
φακζλου υγείασ

Άρκρο 51 (4). Ηλεκτρονικόσ φάκελοσ υγείασ
1. Κακιερϊνεται ο Ατομικόσ Ηλεκτρονικόσ Φάκελοσ Υγείασ για όλουσ τουσ Ζλλθνεσ πολίτεσ
(ΑΗΦΥ).
2. Ο ΑΗΦΥ περιζχει το ςυνοπτικό ιςτορικό υγείασ κάκε πολίτθ, ωσ μζροσ του ιατρικοφ
φακζλου, κακϊσ και τισ πλθροφορίεσ τθσ περίπτωςθσ ιγϋ του άρκρου 3 του ν. 4213/2013.
Με απόφαςθ του Υπουργοφ Υγείασ και μετά από πρόταςθ του Εκνικοφ Συμβουλίου
Διακυβζρνθςθσ Ηλεκτρονικισ Υγείασ (ΕΣΔΗΥ), κακιερϊνεται ενιαίο εθνικό υπόδειγμα −
πρότυπο ΑΗΦΥ ςχετικά με το περιεχόμενο, τον τρόπο κατάρτιςθσ, τθν ταυτοποίθςθ του
ατόμου και τθν πρόςβαςθ ςτισ ιατρικζσ πλθροφορίεσ του φακζλου, ςφμφωνα με το ν.
2472/1997 και το ν.
3. Το περιεχόμενο του ΑΗΦΥ ωσ προσ το ςυνοπτικό ιςτορικό υγείασ, είναι ενιαίο ςε εθνικό
επίπεδο και υποχρεωτικό.
4. Ο ΑΗΦΥ καταρτίηεται από τον οικογενειακό ιατρό ι από το ιατρικό προςωπικό τθσ μονάδασ
υγείασ, ςτθν οποία παρακολουκείται ο αςκενισ. Οι οικογενειακοί και οι άλλοι ιατροί
υποχρεοφνται να τθροφν και να ενθμερϊνουν τουσ ηλεκτρονικοφσ φακζλουσ των αςθενών,
με όλεσ τισ ιατρικζσ πλθροφορίεσ που είναι αναγκαίεσ για τθν παρακολοφκθςθ, τθ κεραπεία
και τθν αποκατάςταςθ των αςκενϊν.
5. Τα δεδομζνα του ΑΗΦΥ αποτελοφν ιδιοκτθςία του πολίτθ και τηροφνται αςφαλώσ, υπό την
ευθφνη του Υπουργείου Υγείασ, ςφμφωνα με τθν ιςχφουςα νομοκεςία περί προςταςίασ
προςωπικϊν δεδομζνων. Πρόςβαςη ςτισ πληροφορίεσ του ΑΗΦΥ του ζχει ο πολίτησ, ο
εκάςτοτε οικογενειακόσ ιατρόσ του, κακϊσ και ο κεράπων επαγγελματίασ υγείασ, κατά τθ
νοςθλεία ι επίςκεψθ του πολίτθ ςε δθμόςια ι ιδιωτικι μονάδα παροχισ υπθρεςιϊν υγείασ
