Recent studies have suggested that the expression of combinations of these lineage-specific transcription factors can convert adult somatic cells from one type to another. Here we report that the combination of p63, a master regulator of epidermal development and differentiation, and KLF4, a regulator of epidermal differentiation, is sufficient to convert dermal fibroblasts to a keratinocyte phenotype. Induced keratinocytes (KCs) expressed KC-specific proteins and had a transcriptome similar to KCs. Reprogramming to a KC phenotype was rapid and efficient with a vast majority of cells morphologically resembling and expressing KC-specific genes within a week of p63 and KLF4 transduction. Furthermore, p63 and KLF4 are capable of inducing a KC phenotype even in a cancerous cell line, highlighting their importance for epidermal specification. The robustness of the conversion process also allows the use of this as a model system to study the mechanisms of reprogramming. tion assay. Results : HSV shedding among placebo recipients was detected on 16.6% of days; shedding among pritelivir recipients was detected on 18.2% of days among those receiving 5 mg daily, 9.3% of days among those receiving 25 mg daily, 2.1% of days among those receiving 75 mg daily, and 5.3% of days among those receiving 400 mg weekly. The relative risk of viral shedding with pritelivir, as compared with placebo, was 1.11 (95% confidence interval [CI], 0.65 to 1.87) with the 5-mg daily dose, 0.57 (95% CI, 0.31 to 1.03) with the 25-mg daily dose, 0.13 (95% CI, 0.04 to 0.38) with the 75-mg daily dose, and 0.32 (95% CI, 0.17 to 0.59) with the 400-mg weekly dose. The percentage of days with genital lesions was also significantly reduced, from 9.0% in the placebo group to 1.2% in both the group receiving 75 mg of pritelivir daily (relative risk, 0.13; 95% CI, 0.02 to 0.70) and the group receiving 400 mg weekly (relative risk, 0.13; 95% CI, 0.03 to 0.52). The rate of adverse events was similar in all groups. We present a case where the melanocyte stimulation of MT-II in combination with the use of sun tanning beds coincided with cutaneous melanoma. Observation : A 20-year-old woman with Fitzpatrick skin type II was referred to a dermatology clinic. Clinical examination revealed a suspicious black melanocytic lesion in her left gluteal region. Furthermore, her skin was universally intensely pigmented. The melanocytic lesion was excised, and histology confirmed the diagnosis of melanoma. Three months prior to the excision the patient had conducted a 3- to 4-week course of self injections with MT-II, intending an augmentation of sunbed tanning. Conclusions and Relevance: This observation brings attention to the potential risks related to the use of the cyclic α-melanocytestimulating hormone analogue MT-II. Methods : Healthy participants were asked to assess the intensity of an experimentally induced itch at their right forearm while they observed externally guided scratch movements either at their right (itching) or left (non-itching) forearm which were either mirrored or not mirrored. In the first experiment, a mirror placed between the participant’s forearms was used to create the visual illusion that the participant’s itching (right) forearm was being scratched while in fact the non-itching (left) forearm was scratched. To control visibility of the left (non-mirrored) forearm, a second experiment was performed in which unflipped and flipped real-time video displays of the participant’s forearms were used to create experimental conditions in which the participant visually perceived scratching either on one forearm only, on both forearms, or no scratching at all. Results : In both experiments, scratching the non-itching limb attenuated perceived itch intensity significantly and selectively in the mirror condition, i.e., when the non-itching forearm was visually perceived as the itching limb. Discussion: These data provide evidence that the visual illusion that an itching limb is being scratched while in fact the non-itching limb contralateral to the itching limb is scratched, can lead to significant itch relief. This effect might be due to a transient illusionary intersensory perceptual congruency of visual, tactile and pruriceptive signals. “Mirror scratching” might provide an alternative treatment to reduce itch perception in focal skin diseases with persistent pruritus without causing additional harm to the affected skin and might therefore have significant clinical impact. 6 Background : Tocopheryl acetate is viscous oily fluid used in a range of preparations for skin and scalp care in Italy. Observational and in vitro data have suggested a high level of efficacy against head louse infestation. The purpose of this investigation was to confirm the activity of tocopheryl acetate in a clinical setting in comparison with a standard widely used preparation. Methods : A spray formulation containing tocopheryl acetate 20% in cyclomethicone was compared with permethrin 1% creme rinse for treatment of head louse infestation in a randomised, assessor blind, trial. Forty-five people were treated on two occasions 7 days apart. The spray was applied to dry hair for 20 minutes then washed. Participants treated with permethrin washed their hair and towel dried it before treatment for 10 minutes. Assessments were made by dry detection combing 1, 6, 9, and 14 days after first treatment. Results : The tocopheryl acetate 20% spray was significantly (p = 0.033) more effective than permethrin 1%, using intention to treat worst case analysis, in which there were 13/23 (56.5%) successful treatments for tocopheryl acetate compared with 5/22 (22.7%) for permethrin. After unprecedented issues of re-infestation within households had been taken into account the underlying cure rate was 17/23 (73.9%) for tocopheryl acetate compared with 5/22 (22.7%), Odds Ratio 9.63 (95% CI, 2.46 to 37.68) (p < 0.001). Department of Dermatology, Columbia University Medical Center, New York Nature Immunology, 2014, 15, 2 T cells autoreactive to the antigen-presenting molecule CD1a are common in human blood and skin, but the search for natural autoantigens has been confounded by background T cell responses to CD1 proteins and self lipids. After capturing CD1a-lipid complexes, we gently eluted ligands while preserving nonligand-bound CD1a for testing lipids from tissues. CD1a released hundreds of ligands of two types. Inhibitory ligands were ubiquitous membrane lipids with polar head groups, whereas stimulatory compounds were apolar oils. We identified squalene and wax esters, which naturally accumulate in epidermis and sebum, as autoantigens presented by CD1a. The activation of T cells by skin oils suggested that headless mini-antigens nest within CD1a and displace non-antigenic resident lipids with large head groups. Oily autoantigens naturally coat the surface of the skin; thus, this points to a previously unknown mechanism of barrier immunity. Figure 1 | Skin-antigen recognition by self-reactive T cells. a, T cells recognize peptide antigens bound to MHC molecules on the surface of antigen-presenting cells, or lipid antigens that are presented by CD1 proteins. In both cases, the antigen protrudes from the groove of the antigen-presenting molecule to engage the T-cell receptor. De Jong et al.1 show that some T cells that react to CD1a molecules (a subset of the CD1 family) recognize oily substances found in sebum — a hydrophobic layer secreted onto the outermost layer of the skin. These self-antigens nestle deep within the CD1a groove, such that there might be direct contact only between CD1a and the T-cell receptor. b, The authors propose that skin-barrier disruption, through trauma or infection, allows Langerhans cells, which express CD1a, to acquire oily antigens from sebum and move from the skin’s epidermis to the dermis. There, they make contact with self-reactive T cells and activate them to produce cell-signalling molecules, such as IL-22, that promote an immune response to barrier disruption without requiring specific recognition of invading microbes. Signaling between epithelial cells and innate immune cells via the cytokine thymic stromal lymphopoietin (TSLP) is thought to drive AD and the atopic march. Here, we report that epithelial cells directly communicate to cutaneous sensory neurons via TSLP to promote itch. Chronic itch development in sensory neurons requires BRAF signaling pathways Objective : Cosmetic dry skin often has a lower hydration level but a similar apparent barrier function, as measured by transepidermal water loss (TEWL), than that of the normal skin. To investigate the intrinsic difference in barrier property and moisture-holding ability between the cosmetic dry and normal skin, we developed a new clinical and data analysis procedure based on sequential tape-stripping with TEWL measurement, coupled with chemical analysis for protein and natural moisturizing factors (NMF) in the stratum corneum. Methods : A clinical study consisting of 64 healthy Caucasian female subjects with normal and cosmetic dry skin was conducted according to our clinical and data collection protocols. After the baseline visual dryness assessment, 20 tape-strips were placed and removed on each test site using D-Squame tapes. TEWL was measured at baseline and after the 5th, 10th, 15th and 20th tape-strips. All tapes were analysed for protein mass via chemical extraction and the Pierce BCA protein assay, as well as using an infrared densitometry device Squame Scan 850A. The stratum corneum thickness and barrier quality (water transport resistance per thickness of the stratum corneum) were decoupled from the apparent barrier function using the TEWL and protein data. Results : A linear relationship between 1/TEWL and cumulative protein removal was observed for both normal and cosmetic dry skin. However, the slope of the linear relation was significantly steeper for normal skin, and significantly more protein was removed from cosmetic dry skin. The results showed that on average, the barrier quality of the stratum corneum of the normal skin is about 40% higher than that of the dry skin, whereas the stratum corneum of the dry skin is about 30% thicker than that of the normal skin. In addition, the amount of SC removal in sequential tape-stripping is generally non-uniform. Conclusion : Our results demonstrated that there are characteristic differences in the barrier property between normal and cosmetic dry skin. In comparison to the normal skin, the stratum corneum of the cosmetic dry skin is considerably thicker, however, with a lower barrier quality. The results also showed that the amount of the SC removal in sequential tapestripping is generally non-uniform. Therefore, the number of tape strips is not a good indicator for the tape-stripping depth. International Journal of Cosmetic Science. We developed a mouse model in which a constitutively active form of the serine/threonine kinase BRAF was expressed in neurons gated by the sodium channel Nav1.8 (BRAF(Nav1.8) mice). We found that constitutive BRAF pathway activation in BRAF(Nav1.8) mice results in ectopic and enhanced expression of a cohort of itch-sensing genes, including gastrin-releasing peptide (GRP) and MAS-related GPCR member A3 (MRGPRA3), in nociceptors expressing transient receptor potential vanilloid 1 (TRPV1). BRAF(Nav1.8) mice showed de novo neuronal responsiveness to pruritogens, enhanced pruriceptor excitability, and heightened evoked and spontaneous scratching behavior. GRP receptor expression was increased in the spinal cord, indicating augmented coding capacity for itch subsequent to amplified pruriceptive inputs. Enhanced GRP expression and sustained ERK phosphorylation were observed in sensory neurons of mice with allergic contact dermatitis- or dry skin-elicited itch; however, spinal ERK activation was not required for maintaining central sensitization of itch. Inhibition of either BRAF or GRP signaling attenuated itch sensation in chronic itch mouse models. These data uncover RAF/MEK/ERK signaling as a key regulator that confers a subset of nociceptors with pruriceptive properties to initiate and maintain long-lasting itch sensation. Methods : A total of 2453 patients underwent patch testing to the NACDG standard screening series. The incidence of positive patch test reaction among patients with AD (n = 342) and without AD (n = 2111) was assessed. Statistical analysis was done using a χ(2) test. Results : Compared with nonatopic patients, patients with AD were statistically more likely to have positive patch tests. AD was associated with contact hypersensitivity to quaternium-15, imidazolidinyl urea, DMDM hydantoin, and 2-bromo-2-nitropropane-1,3-diol but not to parabens, formaldehyde, or diazolidinyl urea. Limitations : Only patients suspected of having allergic contact dermatitis were tested. Our population was geographically limited to metropolitan Kansas City, MO, and metropolitan New York City, NY. Evening Primrose Oil rediscovered Fo c u s Gamma-Linolenic Acid Levels Correlate with Clinical Efficacy of Evening Primrose Oil in Patients with Atopic Dermatitis Introduction : Atopic dermatitis (AD) has been related to a deficiency of delta-6-desaturase, an enzyme responsible for the conversion of linoleic acid to gamma-linolenic acid (GLA). Evening primrose oil (EPO) contains high amounts of GLA. Therefore, this study investigated whether EPO supplementation results in an increase in plasma GLA and its metabolite dihomo-gamma-linolenic acid (DGLA) correlating with clinical improvement of AD, assessed by the SCORing Atopic Dermatitis (SCORAD) index. Methods: The open study included 21 patients with AD. EPO (4-6 g) was administered daily for 12 weeks. Before treatment, and 4 and 12 weeks after initiation of EPO supplementation, objective SCORAD was assessed and plasma concentrations of GLA and DGLA were determined by gas chromatography. Results : A significant increase in plasma GLA and DGLA levels and a decrease in the objective SCORAD were observed 4 and 12 weeks after initiation of EPO treatment. In the per-protocol population (n = 14), a significant inverse correlation between the changes in plasma GLA levels and SCORAD was found (P = 0.008). Conclusion : The clinical disease activity under EPO treatment correlates with the individual increase in plasma GLA levels. Thus, the results of this pilot study indicate that an increase in plasma GLA might be used as predictive parameter for responsiveness of AD to EPO therapy. Advances in Therapy. Epub ahead of print. 10 Moisturizers ignore fillaggrin genes Moisturizing treatment of patients with atopic dermatitis and ichthyosis vulgaris improves dry skin, but has a modest effect on gene expression regardless of FLG genotype. Background : Loss-of-function mutations in FLG (encoding filaggrin) are a predisposing factor for atopic dermatitis (AD) and cause ichthyosis vulgaris (IV). Patients with AD and IV display impaired skin barrier and dry skin, and altered epidermal expression of genes in pro-inflammatory and lipid metabolic pathways are often evident. Objectives : To evaluate the effect of three different moisturizers on skin barrier function and epidermal gene expression in patients with AD/IV in relation to FLG mutation status. Methods: Patients (n = 43) were classified according to their FLG status: AD with FLG+/+ (n = 14), AD with FLG+/- (n = 14), and AD/IV with FLG-/- (n = 15). Dryness score and transepidermal water loss (TEWL) were monitored on volar forearms, and punch biopsies were taken for analysis of gene expression. Measurements were repeated after 4 weeks of treatment with either of two moisturizers on each forearm. Results : Treatment with any of the three moisturizers significantly reduced dryness score and TEWL in the group as a whole. FLG-/- patients displayed the largest reduction in dryness score. Only minute changes occurred in the mRNA expression of 15 selected epidermal genes. Conclusions : Moisturizing treatment improves dry skin and certain aspects of abnormal skin barrier function, especially in patients with AD/ IV and dual FLG mutations, but does not normalize the epidermal gene expression profile. Contact & atopic dermatitis: the complex game The multiple factors affecting the association between atopic dermatitis and contact sensitization Atopic dermatitis and allergic contact dermatitis are both common skin diseases having an immune pathogenesis. There has been considerable interest about their inter-relationships with regard to altered susceptibility. Recent investigations have shed new light on this important question, and in this article, we explore whether there is evidence that atopic dermatitis affects the risk of contact sensitization and allergic contact dermatitis. The use of topical products to treat xerotic and inflamed skin in atopic dermatitis often results in a higher Dermatologica Helvetica – Volume 26(3) – Mars 2014 prevalence of sensitization to, for example, fragrances and other ingredients in emollients. Moreover, the prevalence of metal allergy seems to be increased, probably due to compromised chelation of the metals in the stratum corneum of patients with atopic dermatitis. However, conversely, the T-helper cell 2 bias that characterizes immune responses in atopic dermatitis appears to lower the risk of contact sensitization compared to healthy controls. Based on these observations, we conclude that multiple factors affect the association between atopic dermatitis and contact sensitization, and that these need to be appreciated in the clinical management of atopic dermatitis patients. Allergy. Epub ahead of print. TSLP super star Thymic Stromal Lymphopoietin Variation, Filaggrin Loss of Function, and the Persistence of Atopic Dermatitis. Importance : Atopic dermatitis (AD) is a common chronic illness of childhood. Objective: To evaluate the association between thymic stromal lymphopoietin (TSLP) variation and the persistence of skin symptoms of AD. Design, Setting and Participants: A prospective cohort study was conducted in the general community. Participants included 796 children enrolled in the Pediatric Eczema Elective Registry. Exposure : Evaluation of TSLP variation. Main outcomes and measures: Self-reported outcome of whether a child’s skin had no symptoms of AD and required no medications for 6 months at 6-month intervals. Results: We evaluated 14 variants of TSLP. The variant rs1898671 was significantly associated with the outcome in white children (P = .01). As measured by overlapping CIs, similar odds ratios (ORs) were noted among whites (OR, 1.72; 95% CI, 1.11-2.66) and African Americans (1.33; 0.52-3.45). Further within the subcohort of individuals with a filaggrin protein (FLG) loss-offunction mutation, those with TSLP variation were more likely to have less-persistent disease (OR, 4.92; 95% CI, 2.04-11.86). Conclusions and relevance : The TSLP variation is associated with less persistent AD. Therefore, TSLP may be a potential therapeutic target for the treatment of AD, especially in individuals with diminished barrier function due to FLG mutations. This is an attractive hypothesis that can be tested in clinical trials. Vitamin D deficiency in atopic dermatitis Vitamin D deficiency rickets in an adolescent with severe atopic dermatitis. Atopic dermatitis (AD) affects 10% to 20% of children worldwide. Its severity may be inversely correlated with 25-hydroxyvitamin D (25OHD) levels. Although low levels of vitamin D (VD) can cause rickets in infants, VD deficiency rickets is an unusual presentation in teenagers. We report the case of a 14-year-old girl with severe AD and fish allergy since early childhood. She lived at high latitude (with less sun exposure) and, because of her atopic disorders, avoided sunlight and fish. Laboratory studies showed elevated alkaline phosphatase and parathyroid hormone levels and low serum calcium; her serum 25OHD level was <12 nmol/L. A radiograph of the wrist showed a radiolucent band in the distal metaphysis of the radius with marginal sclerosis. She was diagnosed as having hypocalcemic rickets due to VD deficiency. Treatment with VD increased her 25OHD level to 44 nmol/L, with normalization of alkaline phosphatase, parathyroid hormone, and calcium. Moreover, we observed a dramatic improvement in her AD severity with VD treatment. This case demonstrates the complex interaction between VD deficiency, AD, and food allergy. We advise a high index of suspicion of VD deficiency rickets in children of all ages with AD, particularly during accelerated growth periods and in the presence of other risk factors such as darker skin, living at high latitude, sun avoidance, and low intake of VD-rich foods. The concomitant improvement in bone-related parameters and AD severity may reflect a double benefit of VD treatment, a possibility that warrants research on VD as potential treatment for AD. Darin integriert ist der Nationale Hautkrebstag vom 12. bis 16. Mai 2014 als Schwerpunkt im Rahmen der Frühdiagnostik von Hautkrebs. Cette année, nous avons réorganisé la campagne nationale du cancer de la peau (National Skin Cancer Campaign). Dans ce cadre, nous avons intégré la Journée nationale du cancer de la peau, qui aura lieu du 12 au 16 mai 2014 et dont l’accent principal portera sur le diagnostic précoce du cancer de la peau. SGDV – SSDV Viele Dermatologinnen und Dermatologen äusserten bezüglich der Durchführung des Hautkrebstages in der traditionellen Form ihre Unzufriedenheit. Im vergangenen Jahr haben wir deshalb zwei Umfragen durchgeführt, zum einen bei den Mitgliedern der SGDV, die am Hautkrebstag teilgenommen hatten, sowie eine zweite bei allen SGDVMitgliedern. Bei einer ganzen Reihe von Fragen, z.B. zur Sinnhaftigkeit der Aktion, waren die Meinungen praktisch hälftig geteilt. Bei individuellen Gesprächen mit Kolleginnen und Kollegen haben wir viele konkrete Anregungen erhalten. Basierend auf diesen Informationen sowie den Ergebnissen der Umfrage haben wir das Konzept für die HautkrebsKampagne 2014 neu erarbeitet. Hinzu kommt, dass die Krebsliga Schweiz vor zwei Jahren beschloss, eigene Wege zu gehen und ihren Schwerpunkt nicht mehr im Bereich der Sekundärprävention (Frühdiagnostik von Hautkrebs) zu setzen, sondern sich ausschliesslich auf die Primärprävention (Vermeidung von UV-Exposition) zu fokussieren. Die SGDV und die Arbeitsgruppe Dermato-Onkologie bedauerten dieses abrupte Ende einer erfolgreichen Zusammenarbeit sehr. Die SGDV beschloss daher im Jahr 2012, die Hautkrebsprävention (Primär- und Sekundärprävention) vorläufig selbständig weiterzuführen. Nous avons donc effectué l’an passé deux enquêtes : l’une s’adressait d’abord aux membres de la SSDV ayant pris part à la journée nationale, alors que l’autre était destinée à tous les membres de la SSDV. Sur toute une série de questions, par exemple à propos de la pertinence de l’action, les opinions étaient pratiquement partagées par moitié. Au cours d’entretiens personnels avec des collègues, nous avons reçu de nombreuses suggestions concrètes. Nous fondant sur ces informations ainsi que sur les résultats de l’enquête, nous avons alors élaboré le nouveau concept de la campagne du cancer de la peau 2014. A cela s’ajoute que la Ligue suisse contre le cancer a décidé voici déjà deux ans de suivre sa propre voie et de ne plus mettre la priorité sur le domaine de la prévention secondaire (diagnostic précoce du cancer de la peau), mais de focaliser son action sur la prévention primaire exclusivement (éviter l’exposition aux UV). La SSDV et le groupe de travail Dermato-Oncologie regrettent beaucoup cette fin abrupte d’une collaboration qui s’était avérée fructueuse. En conséquence, la SSDV a décidé en 2012 de poursuivre, provisoirement à son propre compte, la prévention du cancer de la peau (prévention primaire et secondaire). Deshalb geht es nicht darum, die ganze Schweiz kostenlos zu screenen, sondern diejenigen Menschen zu sensibilisieren, die ein erhöhtes Hautkrebsrisiko haben und entsprechend betreut werden sollten. In diesem Zusammenhang sollen auch Ärzte anderer Fachrichtungen für die Problematik Hautkrebs sensibilisiert werden, damit sie Risiko-Patienten als solche identifizieren und einer adäquaten dermatologischen Behandlung zuführen können. Bei der Sensibilisierung versuchen wir neue Wege zu gehen. Wir wissen, dass Männer zwischen 20 und 50 Jahren eine wichtige Risikogruppe darstellen. Sie gehen selten zum Arzt und weisen immer noch eine hohe Melanom-Mortalität auf. Wir haben uns die Frage gestellt, wo wir diese Zielgruppe antreffen können und kamen auf die Idee, dass z.B. bei Sportveranstaltungen wie Eishockey-Spielen zwischen 8‘000 und 12‘000 Personen hauptsächlich aus dieser Risikopopulation anwesend sind. Wir stehen aktuell in Kontakt mit den ZSC Lions und dem EV Zug und hoffen, dass wir bei den Spielern diese Untersuchungen zur Frühdiagnostik von Hautkrebs möglichst bald umsetzen und durch die Medien wirksam publik machen können. Als Variante könnten auch medienwirksame Untersuchungen z.B. bei Politikern durchgeführt und kommuniziert werden. Professional Education der Nicht-Dermatologen In Anbetracht der rasch ansteigenden Inzidenz von Hautkrebs ist es sinnvoll, alle Ärztinnen und Ärzte in die Hautkrebsvorsorge einzubinden. Bei der Professional Education von Nicht-Dermatologen geht es nicht darum, Nicht-Dermatologen beizubringen, wie man Hautkrebs diagnostiziert, sondern ihnen Inhalte allgemeiner Natur zu vermitteln: • • • Welche Patienten haben ein erhöhtes Hautkrebsrisiko? Bei welchen Patienten sollte einmal im Jahr eine klinische Inspektion der Haut durchgeführt werden? Welche Patienten gehören in die Obhut von Dermatologen? Die Mitglieder der Arbeitsgruppe Dermato-Onkologie werden zusammen mit den kantonalen Dermatologen-Gesellschaften und den dermatologischen Kliniken der Schweiz auch Fortbildungsveranstaltungen für Nicht-Dermatologen organisieren. SGDV – SSDV Professional Education der Dermatologen Diese wird wie bisher in den Weiter- und Fortbildungsveranstaltungen der SGDV einen wichtigen Platz einnehmen. Dazu gehören Symposien und Kurse zur Epidemiologie, zur Diagnostik (z.B. der Dermoskopie-Kurs), als auch zur Therapie (z.B. zur nicht-chirurgischen und zur chirurgischen Behandlung von Hautkrebs, zur Targeted Therapy des metastasierten Melanoms oder der kutanen Lymphome), wie sie die Kliniken und die SGDV seit vielen Jahren mit grossem Erfolg anbieten. Sensibilisation des groupes à risque Des travaux menés en Australie ont révélé qu’un screening du cancer de la peau effectué sur une large population prise en général, sans ciblage ni sélection, fait peu de sens. Il n’est donc pas question en l’occurrence d’effectuer des examens gratuits de screening dans toute la Suisse mais, bien plus, de sensibiliser les personnes présentant un risque accru de cancer de la peau et qui devraient être suivies en conséquence. A cet égard, il s’agit également de sensibiliser les médecins d’autres disciplines à la problématique du cancer de la peau, afin qu’ils soient à même d’identifier des patients à risque et de les diriger vers un traitement dermatologique adéquat. Grâce à la sensibilisation, nous tentons d’emprunter de nouvelles voies. Nous savons que les hommes entre 20 et 50 ans représentent un groupe à risque important. Ils vont rarement consulter le médecin et présentent toujours une mortalité élevée due au mélanome. Nous nous sommes posé la question de savoir où nous pourrions toucher ce groupecible et en avons conclu que, dans des manifestations sportives telles que les matches de hockey sur glace par exemple, il y a entre 8 000 et 12 000 personnes qui proviennent essentiellement de cette population à risque. Nous sommes actuellement en contact avec le ZSC Lions et l’EV Zug et espérons pouvoir mettre en œuvre le plus rapidement possible, auprès des joueurs, ces examens pour un diagnostic précoce du cancer de la peau et créer un effet sur le public en recourant aux médias. A titre de variante, des examens dotés d’effets médiatiques pourraient, par exemple être effectués sur des personnalités politiques et faire l’objet d’une communication. Education professionnelle des non-dermatologues Compte tenu de l’incidence – en croissance rapide – du cancer de la peau, il est judicieux de rallier tous les médecins à la prévention du cancer de la peau. Dans l’éducation professionnelle des non-dermatologues, il ne s’agit pas d’apprendre à ceux-ci comment on diagnostique un cancer de la peau, mais de leur transmettre des connaissances d’ordre général: • • • Quels patients présentent un risque accru de cancer de la peau? Quels patients devraient être soumis une fois l’an à une inspection clinique de la peau? Quels patients sont appelés à être suivis par un dermatologue? Les membres du groupe de travail Dermato-Oncologie organiseront, avec les sociétés cantonales de dermatologie et les cliniques dermatologiques de Suisse, des sessions de formation continue à l’attention de non-dermatologues. 14 Dermatologica Helvetica – Volume 26(3) – Mars 2014 Education professionnelle des dermatologues Das Thema der "Patient Education" ist für uns alle noch relativ neu. Dabei geht es darum, die Eigenverantwortung des Individuums zu stärken und die Bevölkerung besser aufzuklären, welche Personen ein erhöhtes Hautkrebs-Risiko aufweisen, wie man sich wirksam vor zu viel UV-Strahlung schützt und der Körper trotzdem genügend Vitamin D3 zugeführt erhält. Auch zu diesem Thema werden die Mitglieder der Arbeitsgruppe Dermato-Onkologie aktiv und gemeinsam mit den kantonalen Dermatologen-Gesellschaften und den dermatologischen Kliniken Patientenveranstaltung organisieren. Auf Wunsch werden Vorträge für die verschiedenen Zielgruppen zur Verfügung gestellt. Celle-ci prendra comme jusqu’ici une place importante dans le contexte des sessions de formation postgraduée et continue de la SSDV. En font partie des symposiums et cours portant sur l’épidémiologie, le diagnostic (par exemple le cours de dermoscopie), et aussi sur la thérapie (par exemple pour le traitement non chirurgical et chirurgical du cancer de la peau, pour la "targeted therapy" du mélanome métastasé ou du lymphome cutané), tels que les proposent avec grand succès depuis de nombreuses années les cliniques et la SSDV. Als Beispiel planen wir in Zürich die Durchführung einer Abendveranstaltung, bei der sich die Patienten vorher registrieren und vor der Veranstaltung online einige Fragen beantworten müssen. Sie können bei der Befragung auch Themenwünsche angeben, nach denen das Programm individuell gestaltet wird. Erst nach Abschluss der Befragung kann sich der Teilnehmer definitiv zur Informationsveranstaltung anmelden. Während der Veranstaltung werden nochmals interaktiv Fragen gestellt und anschliessend mittels TED-System ausgewertet. Es ist vorgehsehen, ca. 6 Monate nach der Veranstaltung nochmals eine Online-Befragung durchzuführen, um herauszufinden, wie nachhaltig die Information war. Le thème de l’éducation du patient ("Patient Education") est encore assez nouveau pour nous tous. Il s’agit en l’occurrence de renforcer la responsabilité personnelle de l’individu et de mieux expliquer à la population quelles sont les personnes présentant un risque accru de cancer de la peau, comment on se protège efficacement d’un excès de rayonnement UV et comment le corps reçoit malgré tout un apport suffisant de vitamine D3. Sur ce thème aussi, les membres du groupe de travail Dermato-Oncologie organiseront activement, en collaboration avec les sociétés cantonales de dermatologie et les cliniques dermatologiques, des manifestations à l’attention des patients. Sur demande, des conférences pour les divers groupes-cibles seront mises à disposition. Sekundärprävention (= Frühdiagnostik von Hautkrebs) Die Teilnahme und die Form der Teilnahme an der Nationalen Hautkrebs-Kampagne ist allen Mitgliedern der SGDV völlig frei gestellt. Wer teilnimmt, kann z.B. einen ganzen Tag, einen halben Tag oder stundenweise an unterschiedlichen Tagen Patienten untersuchen. Die Anmeldung erfolgt online über einen speziellen Link auf www.derma.ch. Die Dermatologinnen und Dermatologen können wählen, ob sich die Patienten ohne Voranmeldung oder auf Termin untersuchen lassen sollen. Ferner können sie entscheiden, ob sich die Patienten ihre Termine direkt über ihre Praxis-Telefonnummer oder über ein Callcenter reservieren lassen sollen. Wenn sich Patientinnen und Patientin über das Callcenter anmelden, wird eine kleine „Hürde“ eingebaut: Es wird ihnen die Frage gestellt, ob der Anrufer bereits bei einem Dermatologen in Betreuung ist. Wenn dies bejaht wird, wird dem Anrufer empfohlen, weiterhin zu den dermatologischen Kontrollen bei seinem behandelnden Arzt zu gehen. Es wird ihm erklärt, dass sich diese Kampagne schwerpunkmässig an Personen richtet, welche noch nie eine Hautuntersuchung bei einem Dermatologen durchgeführt haben. Selbstverständlich können wir nicht ausschliessen, dass Personen ein zweites Mal anrufen und die Frage anders beantworten. Wir haben bewusst diese Hürde eingebaut, um mehr Termine für Leute anbieten zu können, welche es wirklich benötigen. Education du patient (prévention primaire et secondaire) Par exemple, à Zurich, nous prévoyons la tenue d’une session du soir à laquelle les patients doivent s’inscrire au préalable et répondre en ligne à quelques questions avant la session. Ils peuvent également mentionner dans le questionnaire des souhaits quant aux thèmes à traiter, ce qui permet alors de concevoir le programme de manière individuelle. Ce n’est qu’après avoir répondu aux questions que le participant peut s’inscrire définitivement à la session d’information. Pendant la session, des questions seront encore posées en mode interactif et évaluées subséquemment au moyen d’un système informatique. Il est prévu, environ six mois après la session de formation, d’effectuer une nouvelle enquête en ligne afin de découvrir dans quelle mesure l’information a été durable. Prévention secondaire (= diagnostic précoce du cancer de la peau) Tous les membres de la SSDV sont entièrement libres de participer à la campagne nationale du cancer de la peau, et si oui, sous la forme qu’ils désirent. Quiconque participe peut par exemple examiner des patients pendant une journée entière, une demi-journée, voire à certaines heures. L’inscription se fait en ligne via un lien spécial sur www.derma.ch. Les dermatologues peuvent choisir si les patients doivent se faire examiner sans préavis ou sur rendez-vous. Par ailleurs, si le principe du rendez-vous est retenu, ils peuvent décider de les convenir directement par le numéro de téléphone de leur cabinet médical, ou de passer par un callcenter. Dermatologica Helvetica – Volume 26(3) – Mars 2014 SGDV – SSDV Patient Education (Primär- und Sekundärprävention) 15 Eine Woche vor Beginn der Hautkrebswoche bekommen die teilnehmenden Dermatologen die Liste mit den Patientennamen. Sollte diese wider Erwarten nicht gefüllt sein, hat er immer noch die Möglichkeit, Termine an seine Patienten zu vergeben. Ab diesem Jahr wird sich die SGDV wieder an der europaweiten Datenerhebung von "Euromelanoma" beteiligen. Der Patient wird den ersten Teil des Fragebogens ausfüllen. Dann führt der Dermatologe eine klinische Untersuchung des ganzen Körpers durch und bei Bedarf werden ein bis zwei Läsionen dermatoskopisch untersucht. Die Ergebnisse der Untersuchung werden im zweiten Teil des Euromelanoma-Fragebogens festgehalten. SGDV – SSDV Bei der Ganzkörper-Untersuchung kann der Facharzt einen sehr guten Eindruck gewinnen, ob der Patient ein erhöhtes Hautkrebsrisiko hat. In diesem Fall würde er ihm empfehlen, regelmässige dermatologische Kontrollen durchführen zu lassen. Si les patients passent par le call-center, un petit "obstacle" est installé: la question leur est posée de savoir si la personne en question est déjà suivie par un dermatologue. Si la réponse est oui, il lui est recommandé de continuer à effectuer des contrôles dermatologiques auprès de son médecin traitant. Il lui sera expliqué que cette campagne s’adresse avant tout aux gens qui ne se sont encore jamais fait examiner la peau par un dermatologue. Il va de soi que nous ne pouvons exclure que certaines personnes appellent une deuxième fois et répondent autrement à la question. Mais nous avons sciemment placé cet obstacle pour pouvoir proposer davantage de rendez-vous à celles et ceux qui en ont vraiment besoin et qui en profitent le plus. Une semaine avant le début de la Semaine du cancer de la peau, les dermatologues qui y participent reçoivent une liste contenant le nom des patients. Si contre toute attente, celle-ci ne devait pas être entièrement remplie, le dermatologue a toujours la possibilité de donner des rendez-vous à ses patients. Bei kontroll- oder behandlungsbedürftigen Befunden würden wir Sie wie im vergangenen Jahr bitten, ein zweites Blatt auszufüllen und dieses im Anschluss an die Kampagne an das Generalsekretariat zu senden. Der Hintergrund dazu ist, dass wir uns zum Zweck der Qualitätssicherung 6 Monate später aktiv beim Patienten erkundigen werden, ob die Läsion behandelt oder kontrolliert worden ist. Wenn eine Histologie durchgeführt wurde, ersuchen wir den Patient um sein Einverständnis, diese Histologie beim behandelnden Arzt anfordern zu können. A partir de cette année, la SSDV participe à nouveau à la collecte de données à l’échelle européenne, dénommée "Euromelanoma". Le patient remplira la première partie du questionnaire. Die an der Hautkrebswoche teilnehmenden Dermatologen und Untersuchungsorte werden wiederum auf der Website www.melanoma.ch publiziert. Debriefing Lorsqu’il procède à un examen du corps entier, le médecin spécialiste peut se faire une très bonne impression quant à savoir si le patient présente un risque accru de cancer de la peau. Et si tel était le cas, il lui conseillerait de se soumettre à des contrôles dermatologiques réguliers. Last but not least wurde von den westschweizer Kolleginnen und Kollegen zu Recht vorgeschlagen, dass wir im Nachgang zur Nationalen Hautkrebskampagne die Gelegenheit zu einem "Debriefing" organisieren sollen. Dieser Punkt ist zentral und wir werden nach Abschluss der Kampagnen-Woche sämtliche teilnehmenden Ärztinnen und Ärzte innerhalb der SGDV auf elektronischem Weg um die Ablieferung ihrer "Übungskritik" bitten. Dabei sollen die positiven und negativen Punkte der Kampagne zusammengetragen und analysiert werden. Si une histologie a été faite, nous demandons au patient son accord pour pouvoir requérir cette histologie auprès du médecin traitant. Selbstverständlich wird es wieder Anzeigen in nationalen und überregionalen Zeitungen geben, doch die Erfahrung hat gezeigt, dass dies zwar hilft, aber eine individuelle aktive Medienarbeit in der Region nicht ersetzen kann oder ersetzen soll. Wenn Sie in Ihrer Region Medienaktivitäten planen, wären wir sehr interessiert, dies zu erfahren. Dieser Austausch ermöglicht die Erstellung einer Ideen-Börse für andere Dermatologen-Verbände und Mitglieder. Der bisherige Hautkrebstag hat innerhalb der SGDV zu einer lebhaften Diskussion und sogar zu hitzigen Debatten geführt, die uns veranlasst haben, die gesamte Aktion gründlich zu überdenken und zu überarbeiten. Wir möchten an dieser Stelle den Autoren dieser konstruktiven Kritik für ihr ernsthaftes und mutiges Engagement unseren Dank aussprechen. Wir möchten innerhalb der SGDV eine respektvolle Diskussionskultur pflegen, selbst wenn sich unsere Auffassungen nicht immer decken werden. Nous aimerions vous encourager et vous prier de nous soutenir activement dans ce travail avec les médias. Il va de soi qu’il y aura cette année encore des annonces paraissant dans des journaux nationaux et interrégionaux, mais l’expérience a montré que si elles sont certes utiles, elles ne sauraient ni ne devraient remplacer un travail individuel actif auprès des médias dans la région en question. Si vous planifiez des activités médias dans votre région, nous serions très intéressés de les connaître. Cet échange permet la création d’une bourse aux idées pour d’autres sociétés de dermatologues et membres. La Journée du cancer de la peau, telle qu’elle a eu lieu jusqu’ici, a suscité au sein de la SSDV des discussions animées et même de chauds débats, qui nous ont incités à repenser et à retravailler l’ensemble de cette action. Nous aimerions exprimer ici nos remerciements aux auteurs de cette critique constructive pour leur engagement empreint de sérieux et de courage. Bien que les temps actuels favorisent plus la sobriété bureaucratique que les fastes festifs, nous avons souhaité vous proposer le cadre historique et somptueux des salles Rotonde et Sandoz, qui inspirent le calme, la prise de distance et la réflexion face aux pressions du quotidien. Erwachsene Patienten mit mittelschwerer bis schwerer, chronischer Plaque Psoriasis in Monotherapie, bei denen eine systemische Therapie oder eine PUVATherapie angezeigt ist. Psoriasis: 80 mg in Woche 0, 40 mg in Woche 1 und danach alle zwei Wochen 40 mg als subkutane Injektion. KI: Überempfindlichkeit gegen Inhaltsstoffe, aktive Tuberkulose (TB), schwere Infektionen wie Sepsis oder opportunistische Infektionen, mittelschwere bis schwere Herzinsuffizienz (NYHA Kl. III-IV). Hel. 2014;26(3):24-25 Fallbericht Erstvorstellung einer 48-jährigen Frau wegen seit 4 Wochen bestehenden "schwarzen Punkten" in Gesicht, Dekolleté und Achseln beidseits, welche aussehen wie offene Komedonen. Zudem hat sie seit einigen Wochen Bauchkrämpfe nach dem Essen, fühlt sich sonst aber wohl. B-Symptome bestehen nicht. In der klinischen Untersuchung finden sich disseminierte <1mm grosse, schwarze Papeln in Gesicht, Haaransatz, retroaurikulär, Dekolleté, Axillaen und am oberen Rücken. Eine dunkelbraune Macula findet sich zudem an der linken Karunkula. Diese Läsionen zeigen über 2 Wochen keine Grössenprogredienz. Histologisch zeigt sich eine follikelassoziierte, atypische Zellproliferation mit erheblichen Kernatypien und Mitosen. Immunhistochemisch ist diese Zellpopulation reaktiv für S-100 und HMB-45. Der Proliferationsindex (Mib-1) liegt bei 20%. Diese Befunde entsprechen disseminierten follikulotropen Melanommetastasen. Weitere Metastasen finden sich im PET-CT an der linken Karunkula, im Magen, sowie in den Lymphbahnen (axillär, cervikal, hochmediastinal, mesenterial), Lymphknoten (iliakal, inguinal), ossär (Tibia) und im Hirnstamm. Der Primärtumor wird nicht gefunden. Diagnose Follikulotrope Melanommetastasen kanntem Primärtumor, Tx, N3, M1c mit unbe- Management Report Bei positiver BRAF-Mutationsanalyse Beginn der Therapie mit Vemurafenib (Zelboraf ). Hierunter zeigt sich eine Regredienz der Papeln zu Maculae. Im Verlauf treten als Komplikation multiple Keratoakanthome auf. 24 Diskussion Die follikulotrope Metastasierung eines Melanoms ist eine Rarität in der Literatur. Bislang wurden 5 Fälle beschrieben und bei allen war, anders als bei dieser Patientin, der Primärtumor bekannt. Unterschiedlich sind auch die Anzahl der Metastasen, bisher wurden zwischen 1 und maximal 20 beschrieben, in unserem Fall sind es über 50. Differentialdiagnostisch sollte immer an einen melanozytären Nävus, den häufigsten Tumor der Konjunktiva, wie auch an eine primär erworbene Melanose (PAM) und ein Primärmelanom gedacht werden. Seit 2013 besteht neu eine generalisierte Lymphadenopathie. In den Biopsien der Haut und eines Lymphknotens zeigte sich ein dichtes lymphoplasmazelluläres Infiltrat mit einem hohen Anteil an IgG/IgG4 positiven Plasmazellen (Abb. 4-6). Aufgrund der vorliegenden Klinik, der Histologie und Immunhistochemie konnte die Diagnose einer IgG4-assoziierten Erkrankung mit Dakryoadenitis und Lymphadenopathie gestellt werden. Bei der „IgG4-assoziierten Erkrankung“ handelt es sich um eine relativ neu beschriebene, chronischentzündliche Systemerkrankung, die eines oder mehrere Organe befallen kann, ähnlich einer Sarkoidose. Klinisch imponieren tumoröse Schwellungen der betroffenen Organe. Dermatologisch finden sich typi- scherweise juckende erythematöse Papeln, Plaques und subkutane Knoten, welche sich vor allem im Gesicht-, Kopf- und Halsbereich manifestieren. Der Schlüssel zur Diagnose liefert die charakteristische Histologie mit der Trias eines lymphoplasmazellulären Entzündungsinfiltrates, einer storiform angeordneten Fibrose sowie dem Vorliegen einer obliterativen Phlebitis. Dermatologica Helvetica – Volume 26(3) – Mars 2014 und Budenofalk zeigte sich im Verlauf eine Abheilungstendenz, allerdings korrelieren die Manifestationen eines kutanen metastatischen Morbus Crohn‘s häufig nicht mit der Entzündungsaktivität im Darm. Vorstellung mit schmerzlosen und spontanen Ulzerationen und Knoten an den Unterschenkeln beidseits, welche seit 4 Wochen bestehen und an der Zahl progredient sind. Es bestehen keine systemischen Infektzeichen. Klinisch finden sich erythematöse Knoten, Plaques und wie ausgestanzt wirkende Ulzerationen. Histologisch zeigt sich das Bild einer nicht verkäsenden granulomatösen Entzündung mit Riesenzellen, Lymphozyten und Histiozyten. Die PAS- und Gram-Färbungen sind negativ. Die Anamnese, Klinik und die Histologie sind mit einem kutanen metastatischen Morbus Crohn vereinbar, der seltensten Hautmanifestationen dieser chronisch entzündlichen Darmerkrankung. Es handelt sich dabei um eine nicht verkäsende granulomatöse Entzündung wie in den entzündlichen Darmabschnitten, per definitionem allerdings ohne anatomische Verbindung zum Gastrointestinaltrakt. Unter der Einleitung einer Therapie mit Salazopyrin 27 Morbus Paget Maladie de Paget Sir James Paget (1814-1899) Sir James Paget (1814-1899) 3 Krankheiten, die verschiedene Organe betreffen, tragen sein Eponym. Der Lebenslauf dieses aussergewühnlichen Mannes ist geprägt von intensiver Arbeit seit dem 16. Lebensjahr, der ihm aber doch noch zu seiner Lebenszeit grosse Anerkennung erbrachte. Er wurde von Königin Viktoria in den Adelsstand erhoben. Er entstammte einer kinderreichen Familie, war das 5. von 17 Kindern. Das Geld reichte nicht um ihm eine Ausbildung in einem renommierten Etablissement zu ermöglichen und so trat er mit 16 Jahren in die Lehre eines "surgeon" ein. Neben dieser sehr praktischen Tätigkeit mit Patienten beschäftigte er sich mit Botanik und veröffentliche 1834 seine erste Arbeit auf diesem Gebiet mit eigenen Illustrationen. Er setzte seine Studien im St. Bartholomew Spital in London fort. Auf Grund seiner präzisen Beobachtung bei einer Obduktion kam es zu einer grossen Entdeckung. Ungewöhnliche Gebilde im Muskelgewebe erregten seine Aufmerksamkeit. Er untersuchte Proben unter dem Mikroskop und entdeckte den Erreger der Trichinose. 1838 mit 22 Jahren machte er seine Abschlussprüfung und wurde in das "Royal College of surgeon" aufgenommen. Seine prekäre finanzielle Situation verbesserte sich ein wenig, er konnte Vorlesungen halten, Privatunterricht erteilen und bekam eine Anstellung als Kurator im Anatomiemuseum. Seine besonderen didaktischen Fähigkeiten und Beschäftigungen mit mikroskopischen Untersuchungen brachten ihm im Alter von 33 Jahren die Professur für Anatomie und Chirurgie ein. Ab diesem Zeitpunkt ging seine Karriere steil aufwärts. 1851 eröffnete er eine grosse Privatpraxis in London. Die Publikation, die die Dermatologie betrifft erfolgte 1874: "On the disease of the mammary areola preceding cancer oft he mammary gland". Trois maladies concernant différents tissus portent son éponyme. Le curriculum de cet homme extraordinaire est marqué par un travail assidu dès sa 16e année, qui lui a donné durant son vivant une grande consécration. Il a été anobli par la reine Victoria. Il était le 5e enfant d’une fratrie de 17. Pour des raisons de coût, il ne pouvait pas entrer dans un établissement renommé et il commence un apprentissage chez un "surgeon" à l’âge de 16 ans. A côté de ce travail pratique avec des patients, il s’intéresse à la botanique et publie un premier travail à ce sujet avec ses propres illustrations en 1834. Ses études se poursuivent à l’hôpital St. Bartholomew à Londres. En raison de ses observations précises, il fait une grande découverte. Lors d’une autopsie, il remarque des corpuscules inhabituels dans les muscles. En les investiguant sous microscope il met en évidence la trichinose. En 1838, à l’âge de 22 ans, il accomplit ses examens et devient membre du "Royal College of surgeon". Sa situation financière s’améliore un peu, il peut donner des cours et avoir des étudiants privés. Il obtient un emploi dans le musée de l’anatomie. Ses habilités didactiques et ses travaux en pathologie lui valent à 33 ans le professorat en anatomie et chirurgie. Depuis ce moment, sa carrière monte et il ouvre en 1851 un cabinet privé à Londres. La publication qui concerne la dermatologie a lieu en 1874: "On the disease of the mammary areola preceding cancer of the mammary gland". L’osteodystrophia deformans (1876) s’appelle aussi maladie de Paget. Le syndrome de Paget-Schroetter est une thrombose de la V. axillaire et/ou de la V. sous-clavière. Zu sehen sind: • in der unteren Rückenpartie locker verteilt erythematöse Plaques mit urtikariellem Charakter • Im gleichen Areal gibt es ausgedehnte Pigmentationen in retikulärer Anordnung in Schattierung von hell-bis dunkelbraun Diese Präsentation ist typisch für Prurigo pigmentosa Photo du mois Prurigo pigmentosa (P.p.) gehört zu den von Japanern erstmals publizierten Dermatosen. Sie trägt nicht das Eponym seines Erstbeschreibers Nigishima, der den ersten Patienten 1971 publizierte. Sehr rasch folgten identische Beobachtungen aus Japan. Es dauerte aber etwa 10 Jahre bis diese Dermatose ausserhalb Japan diagnostiziert wurde aber die Krankheit scheint in Asien häufiger zu sein [1,2]. Die Dermatose beginnt mit einer erythematösen Eruption, die etwa 1 Woche dauert und von einer auffälligen marmorartigen Pigmentierung abgelöst wird. Da die Erkrankung in Schüben verläuft addieren sich die Pigmentierungen und ergeben Braunfärbungen in netzartiger Konfiguration und unterschiedlicher Intensität. Betroffen sind vorwiegend junge Frauen ohne Begleiterkrankungen. Ausser der Rückenpartie sind andere Körperregionen beschrieben worden wie Hals, Nacken und Brust und Extremitäten. Die Dermatose ist von einem starken Juckreiz begleitet. Andere zu Pigmentierung neigenden Dermatosen wie Lichen ruber planus, Erythema dyschronicum perstans, Kontaktekzem, Erythematodes und Papillomatosis confluens et reticularis de Gougerot et Carteaud kommen als Differentialdiagnose kaum in Frage. Die Pathogenese ist ungeklärt. Die Histologie ist unspezifisch. Es mehren sich Berichte über Patienten die unter Diät und Fasten eine Acetunurie aufwiesen. Es gibt keine Hypothese des Pathomechanismus dieser Assoziation [1,2]. Von Interesse ist eine kürzlich publizierte Arbeit in der die Autoren eine Borrelieninfektion bei Patienten mit P.p. annahmen in Anbetracht einer gewissen Ähnlichkeit der Lyme-Erkrankung und von P.p. bezüglich des chronischen Charakters und der Rezidive [3]. Sie konnten bei 3 von 14 Patienten Borrelien in Hautbiopsien mit PCR nachweisen und eine Borreloninfektion mit dem Nachweis von IgG und IgM Antikörpern im Serum bei diesen Patienten bestätigen. P.p. spricht sehr gut auf Medikamente mit Hemmung der Granulozytenmigration an wie Sulfon (Dapson), Sulfonamide, Kolchizin und Kaliumjodid. In den Publikationen aus Asien werden ausschliesslich Tetrazykline (Minozyklin und Doxyzyklin ) mit sehr guten Resultaten angewendet[4]. Referenzen 1. 2. 3. Sont visibles: • des lésions érythémateuses avec un caractère urticarien réparties sur le bas du dos • dans la même région se trouvent des pigmentations étendues avec une disposition réticulée et de coloration brun clair jusqu’à brun foncé. Cette présentation est typique pour Prurigo pigmentosa Prurigo pigmentosa (P.p.) fait partie des dermatoses décrites par les auteurs japonais. Elle ne porte pas l’éponyme de son descripteur Nigishima qui a publié le premier patient en 1971. Des observations identiques provenant du Japon suivent rapidement. Il faut attendre plus de dix ans pour que cette affection soit diagnostiquée en dehors de Japon. Actuellement, il semble qu’elle soit plus fréquente en Asie [1,2]. La maladie évolue en plusieurs étapes. La phase érythémateuse perdure environ une semaine et est suivie d’une pigmentation marbrée frappante. L’affection est récidivante pendant plusieurs années. Les pigmentations additionnées de plusieurs poussées réalisent une configuration réticulée de tonalité brun clair jusqu’à brun foncé. Les femmes d’âge moyen sans maladie associée sont le plus souvent touchées. Les autres localisations en dehors du dos sont le cou, la nuque, la poitrine et les extrémités. La dermatose s’accompagne d’un prurit intense. Quelques dermatoses ayant une tendance à la pigmentation comme le lichen plan, l’érythema chronicum perstans (dermatose cendrée), la dermite de contact, le lupus et la papillomatose réticulée confluente de Gougerot et Carteaud n’entrent pas réellement en considération dans le diagnostic différentiel. La pathogenèse est inconnue. L’image histologique est non spécifique. Quelques communications font état des observations chez des patients présentant une acétonurie après des régimes et des jeûnes [2]. Il n’y a cependant pas d’hypothèse sur le pathomécanisme de cette association [1,2]. Dans une récente publication, les auteurs ont recherché une infection borrélienne chez des patients atteints d’un P.p., ceci en raison d’une certaine ressemblance de la maladie de Lyme, et P.p. en ce qui concerne la chronicité et la récurrence. Ils ont mis en évidence des spirochètes borréliennes dans les biopsies par PCR chez 3 des 14 patients et confirmé une infection borrélienne chez ces patients par des anticorps IgM et IgG dans le sérum [3]. Le P.p. répond très favorablement aux divers médicaments avec une action inhibitrice sur la migration des neutrophiles comme le Dapson, les sulfamides, la colchicine et l’iodure de potassium.
