SAVE THE DATE Kindergarten & Transitional Kindergarten Round Up Week February 24 24 - February 28 28 (Enrollment forms are accepted throughout the year.) Kindergarten Round-up is a pre-registration time for the 2014-2015 school year. Age Requirements: Kindergarten EntryEntry- Children that turn 5 by September 1. Transitional Kindergarten “TK”“TK”- Children that turn 5 between September 1 and December 2. The following Birth Certificate and Health information is required to complete registration: • Original birth certificate provided by the County Registrar/County Recorder or passport • Verification of immunization against: o diphtheria, tetanus and pertussis (DTP) o polio (OPV or IPV) o measles, mumps, and rubella (MMR) o hepatitis B o varicella (chickenpox) • Dental screening(forms will be available at time of registration) Proof of ResidencyResidency- two original items are required to complete registration. Refer to list below for acceptable documents to use for proof of residency: • Utility bills, contracts, statement or payment receipts w/address (cell phone bills not accepted) • Lease/Escrow /Rental papers w/address • Property Tax payment receipts • Pay stub • Voter registration • Correspondence from a government agency Note: Parents have up to two weeks from the start of school to provide proof of immunizations or to complete a waiver based on religious and/or medical beliefs or students will be excluded from school.
