Students must be currently in the 9th grade in your band program.
The Director must be a current member of the FBA and attend the FBA Summer Convention if students are selected.
The Director must submit a brief description for each student nominated in the space provided on the second page of this
nomination form. For each student, please list information about their musical aptitude, achievements, etc. Consideration will
also be given to each student's solo/ensemble evaluation(s), their band's MPA evaluation(s), as well as other musical honors/
achievements, etc.
The Director may nominate UP TO FIVE STUDENETS, ranked in order of performance ability. Please include color instruments
in your selection.
The Nomination Form DEADLINE is (postmarked by) April 1, 2015. Incomplete, incorrect, or late nomination forms will not
be considered.
A $10.00 fee per school will be sent upon notification of accepted students INTO the ensemble. Only school/booster checks or
money orders will be accepted (payable to the Florida Bandmasters Association or FBA). PERSONAL CHECKS WILL NOT
BE ACCEPTED!!!
All accepted participants must complete and return a Nine Star Honor Band Participant Contract found on the FBA web page.
