TODAY: Turn in your Fall - Spring meal plan contract along with this Meal Plan Deferment Request and a $75 nonrefundable fee and $25 deposit ($100 total) for each semester. 2. August 22, 2014: Complete this Meal Plan Deferment Request form and bring it to the Office of Student Financial Assistance (UCF Dining Services will keep the attached copy). Complete this step by Friday, August 22, 2014 for the fall semester and by Friday, January 16, 2015 for the spring semester. By failing to provide a signed release of financial aid information to the Office of Student Financial Assistance, your meal plan payment will no longer be deferred and full payment will be due at that time. 3. September 12, 2014: You must pay the remaining balance of your meal plan by this date (less the $25 deposit). Complete this step by Friday, September 12, 2014 for the fall semester and by Friday, February 6, 2015 for the spring semester. If full payments are not submitted at this time, a $50 late fee will be added to the balance of the meal plan each week payment is not received. Please note: if you have not yet received your financial aid refund by this time, please contact the UCF Dining Services office for further assistance at 407-823-2651. Deferment Request - Terms and Conditions I, the meal plan holder, am requesting deferment of my Fall 2014-Spring 2015 meal plan until Friday, September 12, 2014 for the Fall semester and until Friday, February 6, 2015 for the Spring semester. I acknowledge that this is a legally binding document and a two semester commitment. By signing this agreement, my signature below indicates that I understand and agree to the following: 1. 2. 3. 4. 5. 6. All meal plan holders are signing up for a two-semester contract. Those who request to defer agree to complete this paperwork for both the fall and spring semesters. All deposits ($75 non-refundable fee + $25 deposit) are due at the time of sign-up for fall semester and again for the spring semester, along with deferment request paperwork and meal plan contract. Meal plan holders must sign and complete the release of financial aid deferment funds below to the Office of Student Financial Assistance by Friday, August 22, 2014 for the fall semester and by Friday, February 6, 2015 for the spring semester. The meal plan holder must pay all balances owed by September 12, 2014 for the fall semester, and by January 30, 2015 for the spring semester. The balance will reflect the total price of the meal plan, less the $25 deposit. Failure of payment could result in loss of certain privileges such as the meal plan holder’s account being placed on hold. This prevents use of the meal plan and could hinder use of campus facilities and class registration. If payment in full is not received by September 12, 2014, a $50 fee will be incurred on a weekly basis. Financial Aid funds are not paid directly to UCF Dining Services. It is the patron’s responsibility to pay the remaining balance directly to UCF Dining Services in Ferrell Commons #109 by the due dates listed above. I acknowledge that I have read the terms and conditions of the UCF Dining Services financial aid deferment request. By signing this, I authorize the Office of Student Financial Assistance to release my award information to UCF Dining services for the purpose of determining if I qualify for this deferment. If it is determined that I do no qualify, I understand that I am responsible for paying the full cost of the meal plan to UCF Dining by the aforementioned date.
