Compensation Acknowledgement for Premium (Premium 手数料承諾書)
NOTICE: the purpose of this document is for you, the client, to advise FXDD MALTA LTD as to and to
acknowledge the compensation you have agreed to pay for using the Premium platform. This document also
advises you than in some instances FXDD MALTA LTD may pay a portion of the commission charges to
your Introducing Broker and/or your Trading Agent. In these instances you should also be aware of the
potential for a conflict of interest as further set forth below.
下記に署名を行った顧客である私 _____________________（お名前：ローマ字）は、下記に定義された次の手
数料が Premium 口座を使うためにアカウントに請求されることに同意し、承認します。請求される手数料の金額を
横にあるスペースにお書き込みください。手数料が請求されない場合は空白のままにしておいてください。
a) _$5.98 / 598 円__ Dollars per standard lot round turn traded for commission

I hereby specifically acknowledge and agree that there is an inherent conflict of interest in paying my
Trading Agent or Introducing Broker a per trade commission. The more trades made in my account the
more commission I will pay regardless of the profit or loss on those trades.

I hereby indemnify and hold harmless FXDD MALTA LTD against any and all losses, costs and expenses incurred
in my account and/or errors made by my Introducing Broker or Trading Agent.
