ざいます。・規定の書類（入園申込書、メディカルフォーム、予防接種証明書、等）と本規約書にご署名をいた
だき保育費等のチェックと共にお申し込みください。必要な書類がそろわないと入園、登園出来ない
場合がありますことを御了承ください。・９月から翌年６月までを１学年とし保育を行います。学年途中からのお申し込みについてはご相談
ください。・新規で入園される方は、登園開始月の保育費・入園登録費・デポジットをお預かりさせて頂きます。・デポジットは、１ヶ月分の保育費をお預かりさせて頂きます。規定に基づく退園方法で未払いの保
育費がない場合に、退園後一ヶ月以内にお返し致します。・園生活の写真を新聞やウェブサイトなどに掲載する事がありますことをご理解ください。公共機関
への掲載を控えたい場合には入園時にお伝えください。（別途承諾書参照）・スターチャイルド近くの公園へ出かけたり、バスなどの公共交通機関を利用して、園外に散歩へ出
ることをご理解ください。（別途承諾書参照）・毎月お渡しする月案、週案の保育テーマの中で、宗教に関わる行事がはいることがあります。差し
障りのある場合にはご連絡ください。・入園後の登園スケジュール変更に関しては、可能な限り対応させて頂きますがご希望に添えない場
合がございます。お休みについて・いかなる理由の欠席についてもお申し込みいただいた後の返金、メークアップはいたしませんので、
ご了承ください。・長期休暇について：１ヶ月以上のお休みを長期休暇とし、休暇１ヶ月以上前にご相談いただいた場
合は対応を考慮させていただきます。保育費について・入園時に頂く入園金に関して、一旦退園後はいかなる場合でもご返金はいたしかねます。・保育申し込みは申込書に必要事項をご記入の上、保育費と共に前月２０日までにデイケアにて手
渡し、又はデイケア住所まで２０日必着で郵送してください。お支払いを完了頂いた時点で、お
席の確保を致します。
・月の基本保育費については規定のホリディ（休園日）、月に４週、５週に関わらず申込書にある一
律の料金とします。・保育費は、１０ヶ月、５ヶ月、３ヶ月、１ヶ月ごとの支払いが可能です。詳細は別途入園申込書を
ご覧下さい。・保育費は、チェックもしくはキャッシュにて申し受けます。（チェックの宛名：Starchild）・支払いが２０日を過ぎた場合は＄５０申し受けます。・いただいたチェックが不渡りの場合は、保育費に加えて銀行手数料＄５０を申し受けます。・兄弟姉妹でお申し込みの場合２人目以降保育費のみを１０％引きとします。・保育費は改訂されることがあります。・追加保育について：お席に空きがある場合＄１５／１時間（キッズクラス）、＄２０／１時間（ベビークラス）でお受け致します。退園について・退園の際は１ヶ月前に書面にてお知らせください。・セメスター途中の退園の方で、お支払い済みの保育分が残っている場合のご返金は原則致しかねま
す。残る保育費はスターチャイルドグループでご利用頂ける授業料としてクレジットとさせて頂きま

The daycare might refuse the admission if the child would not be qualified suitable for our programs. ・ We have Baby Class (Basically for under 2 years old) and Kids Class (Basically over 2 years old). After the trial, a child around 2 years old will be chosen for either one of the classes which is qualified suitable by our estimation. ・In order to enroll your child/children in our programs, please submit all of the papers including the medical forms, the immunization letter, the signed application form, and the check/cash of the tuition. The daycare might refuse the admission and attendance if those documents are not submitted. ・ The daycare program starts from September and ends in June of the next year. Please consult with us if you would like to apply during the term of the semester. ・We will charge the tuition of 1 month as Deposit for reserving a new seat. This deposit will be returned within a month after leaving our program only if there is no unpaid fee/tuition and you leave by following our regulation in the right way. ・Please understand that we might use some photography of children for advertisement purposes on News Paper, Website, etc. If you would not like your child/children’s photography to be exposed in public, please inform us when you register. (Please see the attachment.)
・Please understand that we sometimes take Public Transportation such as Bus and Subway for our outside activities. (Please see the attachment.)
・There is a chance that we might sometimes pick a religious topic such as Easter, Christmas, Hanukah, etc for a weekly theme but we do not talk about God or the religion itself deeply. If you have any problem with it, please inform us in advance. ・There is a possibility that changing the attendance date might be difficult after once you enrolled in the program. About the Absence ・We will NOT accept any make-‐up or refund for the absence with any reasons. ・Please inform us at least a month before if you will be away for long vacation of more than a month. Then, we will consider the case. About the Tuition ・The Daycare charges the Registration Fee for enrollment. The Fee is not refundable. ・Please make a payment by 20th of the previous month. We will reserve your seat when you complete the payment. ・The Monthly Tuition does NOT vary due to Holidays or the Number of Weeks. ・The payment could be Monthly, every 3 Month, every 5 Month or 10 Month. ・Please make a payment by Check or Cash. (Payable to: “Starchild”) ・There will be $50 late Fee for the Past Due of 20th of each month. ・The Daycare charges $50 for a Bounced Check. ・10% Discount will be applied for a Sibling’s Enrollment. ・Please understand that the tuition may vary. ・$15/1hr for Kids Class and $20/1hr for Baby Class are the Extended Daycare Service and this is only available when there is an opening seat. About Leaving Our Program ・Please inform us with a Written Notice if your child leaves the Daycare at least a month before ・We would not be able to refund any of the paid tuition; instead, we would use the remaining as your credit for any of classes of Starchild (Daycare, Music Class, Terakoya, After School Program, etc).
