Advocacy Center Mobile Medical Clinic offers well child
by parent or legal guardian.
physicals and immunizations to children 18 or under and:
Appointments are required, please call.
 Are enrolled in Medicaid
 OR have no health insurance
Don’t wait
 OR are American Indians or Alaskan Natives
until the last
 All children must come with their legal guardian

 The legal guardian must bring a photo ID
ready the next school year.
 EVERY child must come with an immunization
record (whether they need shots or not)
HEARING: St. Joseph’s Children’s Advocacy Center will be
conducting hearing screenings for children ages birth-18.
There is no charge ($25 value) and no appointment is
necessary.

a well child physical,
hearing & vision
screening.
VISION: Prevent Blindness Florida will be providing
vision screening (a $23 value) for children ages 2-18.
No appointment is necessary. To those who qualify,
assistance will be provided for glasses and exams.
KidCare Enrollment Available, ask the staff.
Children’s Board Family Resource Centers are administered by the Healthy Start Coalition of Hillsborough County.
Managing partners include: Champions for Children, The Children’s Home, Inc., Family Community Advisory Councils,
REACHUP, Inc. and St. Joseph’s Children’s Hospital Children’s Advocacy Center.
All programs and services are at no charge to families and individuals.

CLINICA MOVIL DE SALUD: La clínica móvil del St. Joseph’s
Children’s Advocacy Center ofrece exámenes de salud y
vacunas a niños menores de 18 años:
 Que estén inscritos en Medicaid
 O que no tengan seguro medico
 O que sean nativos de Alaska o Indios Americanos
 Todos los niños deben de venir acompañados de un
tutor legal
 El tutor legal tiene que traer identificación con fotografía
 CADA niño tiene que traer su record de vacunas necesiten
vacunas o no)
PRUEBA DE AUDICION: Es muy importante realizar un examen de audición a los niños tan pronto sea posible. St. Joseph’s
Children’s Advocacy Center estará realizando examenes auditivos para niños recién nacidos hasta los 18 años. No hay costo
para el examen (tiene un valor de $25) y no necesita cita.
