1. If this is a missing infant or child and/or a suspected abduction, lock down the unit.

b. If you cannot lock the exits, post staff at each exit.

2. Write down a description of the missing patient. Include the following:

3. Write down a description of the abductor (if this is a suspected kidnapping).

4. Call __________to report
a. Describe the missing person (and kidnapper if an abduction), using the description(s) you wrote
down.
b. Tell Securiy where the missing person was last seen.
c. Tell Security how long the missing person has been missing.

5. Call the Operator and( ask for an overhead announcement with the following information:
a. The Code type (eg code pink)
b. The location the person was last seen
c. Describe the missing person (and kidnapper if an abduction), using the description(s) you wrote
down.

6. Call pager to report to the Hospital Administrator On Call (AOC) or the Administrative
Coordinator (AC) and pager to report to the Administrator On Call.
7. Notify the missing patient’s attending physician.

8. Tell staff in the area that a Missing Person Code has been initiated on your unit and that they will need to
stay until released by Security
9. Designate one staff person as the liaison with Security.
[ ] 10. Designate one staff person as the liaison with the patient’s family, if family is present. (This staff person will
provide the family with updates on the search.)
[ ] 11. If this is a missing infant or child and/or a suspected abduction, tell the staff person who is the family’s
liaison to move them to another room. Secure the room where the patient was last seen in order to
preserve physical evidence for Security and other law enforcement officers.
[ ] 12. If the family is not present at the time of the incident, work with Security to notify the family.
[ ] 13. Determine if there is a need to contact a social worker to help the family.
[ ] 14.Call ________ to alert News & Public Affairs about the missing person so that they will be prepared
to deal with the local news media.
[ ] 15.If this is an infant, secure the infant’s medical records and initiate appropriate measures to obtain the cord
blood sample and secure the infant’s photo (if any).
[ ] 16.Assist Security as needed.
