ACTIVITY CLUB for Spring 1 2015

Listed below are the after school clubs that will be available for the first of the Spring 2015 week commencing Monday 12 January for
5 weeks. If your child would like to attend one of these please sign up below with payment attached. These must all be booked via the
office.
All activities are held from 3.45pm to 4.45pm and prices are per half term subject to the number of sessions. Your child may bring a
healthy snack to eat at the end of the school day. Children will be dismissed by the individual coaches/instructors promptly at 4.45pm.
As always after school care with RAPS stay and play is bookable through Julie Surplice.

Please note: It may be the case that your child will be asked to play in various tournaments. This will be classed as part of their training
session therefore refunds will not be given. If this is the case your child will receive a permission slip nearer the time.
Places are allocated on a first come first served basis so please sign up as soon as you can.

Important: Payment must be made in advance. You may either pay by cheque/cash or make an on-line payment, please ensure that you allocate the
payment to the correct club i.e. (2015 Spring 1 Infant/Junior Football, Gymnastics or Dodgeball etc.) Please make cheques payable to HCC. Any
outstanding monies from previous clubs may result in your child not being able to take part again.
