Dear Skiers, Boarders, and Parents:

In addition to forms, you will find updates, ski weather condition, and announcements.
Participation in Ski Club requires a 2.0 or above grade average
Deadline dates for returning completed forms are as follow:
MAD RIVER: Monday, Dec. 8
READ the following information very carefully before signing up for Ski Club:
We will be skiing evenings at Mad River Mountain. Our first trip will be Tuesday,
January 6, 2015. All the ski trips will be on Tuesday evenings.
All Skiers Please Note: The Ski Club payments are divided between the school and
the Ski Area (Mad River). The ski area portion sign up is done online.
Bus departure for ski slopes is 3:00p.m.from the high schools. Departure and return will
be on the north side (stadium side) of the building at Coffman, Brand Rd. parking lot at
Jerome, and the east side at Scioto. The buses return between 10:15p.m.and10:30p.m.
from Mad River. It is imperative that the skier is picked up on time. No one is available
to enter the buildings. If chaperones and students have to wait for someone to be picked
up, that student will not be permitted to go the following week. We do not expect parents
to have to wait for students returning; therefore we do not expect chaperones to have to
wait for parents to pick up students. Students are not to delay bus departure from the
slopes.
NOTE: Due to weather or other circumstances beyond our control, a minimum number
of visits and/or lessons are not guaranteed or implied. The program starts with one visit
and then up to five or more possible visits. Providing weather cooperates, we plan to go
six weeks. If we miss a time, that day will be added at the end. The decision to cancel a
trip will be made prior to 12 o’clock noon on that day. IF SCHOOL IS CANCELLED,
THE SKI TRIP IS ALSO CANCELLED!!
SKI CLUB DATES
MAD RIVER: Tuesdays: January 6, 13, 20, 27 February 3, 10
Make Up: February 17, 24, March 3,10
*** A second trip in a given week may be necessary because of weather or other
circumstances beyond our control.
Students may sign up for any bus they choose, but once the selection is made and lists
printed, no changes will be made. The ski buses are chaperoned by adults, at least one
per bus.
Please be advised that we will be sharing Ski Club expenses and buses with other schools
in the Dublin City School district.
ALL SKI FORMS (3) must be turned in for the Skier or Boarder to participate:
Medical Authorization Form (must be notarized)
Ski Club Permission Form
Co-Curricular Activity Code Agreement Form
CHECKS MADE PAYABLE TO: Coffman High School, Jerome High School, or
Scioto High School (Please put skier’s name on check memo)
The Ski Club dues include transportation and the right to use a ski program ID card to
obtain lift ticket, equipment rental, and/or ski lesson according to the program chosen.
The choices are optional for your needs.

MAD RIVER
The Mad River portion of the sign up process will be done online using the
Mad River Website and a credit card. See instructions below for details.
Username and Password are issued when Ski Club forms are returned to the Advisor
The School portion for joining the Ski Club and riding the Ski Club bus to and from
Mad River is paid to the school. Ski Club Dues: $135

Log On to www.skimadriver.com. “You need a credit card for your registration”
On the Mad River home page, find the words: “Ski Club Login”
Click on the words: “Ski Club Login”
Enter your school name where it ask for Club Name (use one word school name only)
Enter the password given by the Ski Club Advisor in the space provided.
Follow on screen prompts to complete your registration.
Print your receipt when finished.
No ski pass can be issued until Ski Club dues are paid
and all forms turned in
SKI DAY EQUIPMEHT STORAGE
Coffman: No storage area at the school. Students are asked to keep equipment in
their cars or have a parent bring it after school. Independent arrangements can be made
with some teachers. Please ask those teachers in advance.
Jerome: Equipment to be placed in the storage room on the Guidance Office side
of the Commons Area.
Scioto: Equipment to be placed in Room A-1.
SKIING /BOARDING INFORMATION
All participants must have a photo ID picture taken by the Mad River personnel.
Any ski forms turned in after December 8, 2014 must pay $50 late fee.
No ski forms or money will be accepted after Jan. 6, 2015.
Each year students lose equipment at the ski slopes. Ski Check is available to secure
equipment. Ski equipment should be locked when not in use. At the very least, skis
should be separated when placed in the racks. Students who rent equipment will be given
a number. This number must be memorized. All equipment is assigned based on this
number.
ONCE A WEEK SKI PASSES are issued to students each week as they get off the busses and
collected when they re-board the buses. If a skier is not able to participate on a given night, the
ski pass cannot be sold or given to another skier.
HELMETS: Skiers and Boarders should wear a helmet as a safety precaution when on the
slopes at ski area. (See attached note titled “Helmets and Wrist Guards”)
WRIST GUARDS: Snowboarders should purchase wrist guards to wear when on the slopes
at the ski area. (See attached note titled “Helmets and Wrist Guards”)
SKI CLUB RULES
1.
ALL students must ride up on the bus and return on the bus.

Students may not ride home with another student or parent.

ALL SCHOOL RULES APPLY.

NO DRUGS, ALCOHOL, OR TOBACCO!

Buses are locked until 9 p.m.

ALL PROGRAM PRIVILEDGES are subject to revocation without refund for violation of
school, ski club, or ski area rules. Dublin Coffman, Jerome, and Scioto High Schools are proud
of our students and their behavior. We know that our continued good behavior will enable us to
maintain this level of pride.
MEDICAL FORM
It is extremely important that the medical form is completed accurately and the signatures
included. This gives us permission to allow medical personnel to act in an emergency. We will
make every possible attempt to contact you in the event of an injury. If the injury is serious
enough, the student will be transported to hospital that can provide the appropriate care. Every
attempt will be made to contact you before any decision is made as to what should be done for the
student. We have not had a serious injury since the Ski Club has been organized, but we do want
to be prepared with necessary information if a need.
A notary public must witness the signature on the medical form.
We are looking forward to a great winter season. Hopefully we will have good snow and weather
conditions at the slopes.

understand that violation of School, Ski Club, or Ski Area Resort rules can mean
expulsion from the club, loss of monies paid, and possible disciplinary action from the

SATURDAY SKI TRIPS
To Holiday Valley, Snowshoe, and Boyne Mountain
The school sponsored Ski Clubs at Coffman, Jerome, and Scioto will be sponsoring three special
weekend ski trips. The first ski trip is to Holiday Valley in Ellicottville, New York on Saturday
January 24, 2015. The second ski trip is to Snowshoe in Snowshoe, West Virginia on Saturday
February 7, 2015. The third ski trip is to Boyne Mountain in Boyne Falls, Michigan on Saturday
February 28, 2015. Skiers may participate in the regular Tuesday program as well as going and
these trips. Skiers can decide to go on one or all the ski trips to Holiday Valley, Snowshoe, or
Boyne Mountain. You may participate in all the trips without attending the regular ski club
nights. However you must join the Ski Club and fill out the necessary forms to participate in
these special Ski Club activities. Interested participants must complete the following forms: Ski
Club Application Form, Notarized Medical Form, School Code of Conduct Form, and
Special Ski Trips Permission Form.

The cost includes transportation and lift ticket and water park ticket at Boyne Mountain.
Equipment rental and food costs are extra.

If you wish to participate in the trips, fill out the appropriate ski club forms and the special trip
permission form. Space is limited, so we need your check and forms by January 14, 2015.
Skiers can continue to signup for the trips, after the deadline, if seats are available on the bus.
Weekly skiers need to make a separate check out for these special trips.
