Social Worker 1.0 FTE position
(Westmount Family Physicians (combined 0.6 FTE regular + 0.4 FTE contract)
The Thames Valley Family Health Team is one of the largest family health teams in Ontario. With more than 100
physicians across 16 sites within London, and Middlesex, Oxford and Elgin counties, the Thames Valley Family
Health Team is committed to providing comprehensive inter-collaborative primary health care to over 155,000
patients. Over the past five years, our affiliated physicians have taken on more than 22,000 Ontarians who did not
previously have a family doctor.
Family Health Team staff includes nurse practitioners, nurses, social workers, pharmacists, dietitians and other health
care professionals who will work with physicians as an interdisciplinary team, each using their experience and skills to

The Social Worker provides assessment, evidence-based treatment, and ongoing management of biopsychosocial
aspects of health and well-being for identified patients rostered to the physicians of the Thames Valley Family Health
Team (TVFHT). The Social Worker participates in the development, implementation, monitoring and evaluation of
programs to meet the needs of the TVFHT patient population. Working in collaboration with an Interprofessional
Healthcare Provider (IHP) team, the Social Worker assists with health promotion, disease prevention, and chronic
disease management across a patient’s lifespan. The social worker uses current best practice guidelines to monitor
client goals and outcomes, assists with system navigation, provides education, & promotes client self-management.
Requirements:






Masters or Undergraduate degree in Social Work from a recognized university or an equivalent program
Excellent clinical skills and minimum 3 years’ experience assessing and managing mental health conditions
commonly seen in primary care.
Proficient in assessment, & a variety of counseling techniques for working with individuals and families
Current registration with the Ontario College of Social Workers and Social Service Workers
Proof of professional liability insurance
Ability to meet site-specific requirements
Skills/Abilities:












Experience related to the delivery of efficient primary care, including the ability to share information and
teach
Knowledge and proficiency in current, evidenced-based methods and practices of social work in primary
care settings, with an emphasis on chronic disease management, health promotion and risk reduction
Monitors client goals and outcomes through ongoing assessment and documentation of treatment progress
Experience facilitating groups an asset
Knowledge of relevant health care legislation and community mental health resources
Understanding of social determinants of health and their impact on health and well-being
Strong leadership, organization, research, evaluation, time management and interpersonal skills
Experience and commitment to working both independently and collaboratively with other IHP’s
Desire and ability to update knowledge and skills through various means including technology-based
opportunities, courses, workshops and conferences
Experience in the use of EMR, Microsoft Word, Excel and Outlook
Demonstrated ability to be open and non-judgmental
Excellent verbal and written communication skills
SALARY RANGE: $28.33 – $35.3861/hour
