Course Outline
This course aims to introduce valuable safety management lessons derived from major accident
inquiries in Singapore. This is an important part of Singapore’s engineering history and, it is
necessary to capture the lessons in the form of case studies so that they can be shared with current
and future managers, engineers, supervisors, technicians and workers. The recently published book
by Goh and Soon (2014) describes three major accidents in Singapore and their safety management
lessons. This seminar will introduce some of the chapters from the book and extends the discussion
further. The discussion will focus on incident analysis techniques, the Nicoll Highway collapse case
study and discussion on topics such as design, organisational structure, culture, and major hazards
monitoring.
The aim of the discussion is not on the legal and liabilities aspects of the major accident. Instead the
focus is on learning from past the accident (based on the details in the Committee of Inquiry report)
so as to improve safety management and prevent future major accidents.
Course objectives:
By the end of the course, participants will be able to:
• Have an appreciation of the Event Causation Technique
• Understand the circumstances of the Nicoll Highway accident
• Understand the warning signs of a deteriorating hazard monitoring system
• Understand the importance of systems thinking concepts in accident prevention
• Appreciate the need to adopt a risk management approach when dealing with design changes
• Understand how structure, processes and culture can mould the behaviour and decisions of
individuals

Profile of speaker
Dr Goh is an Assistant Professor with the National University of Singapore. He currently leads the
Safety and Resilience Research Unit (SaRRU) in the Centre for Project Management and
Construction Law. At the same time, he is the Deputy Programme Director for the Bachelor of
Project and Facilities Management in the Department of Building, NUS. Dr Goh specialises in
Workplace Safety and Health (WSH) and risk management. He is appointed as an Honorary
Researcher of the Workplace Safety and Health Institute in Singapore (2014-2017). He worked as a
senior consultant in the oil and gas industry, Assistant Director (Investigations) at the Singapore
Ministry of Manpower, and Senior Lecturer at Curtin University in Western Australia. Dr Goh is
currently an Adjunct Associate Professor with Curtin University and an approved Workplace Safety
and Health Auditor.the various professional organizations such as Singapore Structural Steel Society
(SSSS), Singapore Quality Institute (SQI), Singapore Welding Society (SWS), Institution of
Engineers, Singapore (IES) and BCA Academy.

TERMS & CONDITIONS COURSE REGISTRATION
Registration
Any registration, whether on-line or fax will be on a first-come-first-served basis and will only be
confirmed upon receipt of full payment by The Institution of Engineers, Singapore (IES) unless
otherwise invoice to company.
All registrations must be submitted with duly completed registration form.
Email registrations will not be accepted.
Closing Date & Payment
The closing date of the event will be 7 days prior to event commencement date. Cheques should be
crossed ‘A/C payee only’ and made payable to ‘Institution of Engineers, Singapore’ or ‘IES’, with
the Date and Title of The Event indicated clearly written on the back of the cheques, and
submitted with the duly completed registration forms to:

Confirmation of Registration
Confirmation of registration will be given 7 days prior to the commencement date via email. If you do
not receive the said confirmation email, you are required to contact IESA general admin immediately at
6463 9211 (office).
IESA reserves the right to allow only confirmed registrants to attend the Event.
Withdrawals/Refunds of Fees
Notice of withdrawal must be given in writing to IESA. Policy on refund of course fee is as follows:
FULL refund if we receive your written notice of withdrawal at least 7 days before the
commencement of the Event.
NO refund otherwise.
No show of participant would not be accepted as reason for withdrawal/refund.
Replacement is allowed but restricted to once only. Replacement will be allowed only if written notice
is received by us at least 3 days before the commencement of the event. However, when an IES
member is replaced by a non-member, the participant has to pay the difference in the relevant fees at
least 3 days before the commencement date.
Cancellation/Postponement
Changes in Venue, Dates, Time and Speakers for the Events can occur due to unforeseen circumstances.
IES reserves the full rights to cancel or postpone the Event under such circumstances without prior
reasons. Every effort, however, will be made to inform the participants or contact person of any
cancellation or postponement.
Fees will be refunded in FULL if any Event is cancelled by IESA.
