As a volunteer of the JDC, I agree to observe the following Code of Conduct: I will be an ambassador for the tournament, our title sponsor, our community and new volunteers. I will display control, respect, courtesy, and professionalism to all involved in the tournament, including but not limited to, not requesting autographs or pictures and/or initiating conversation with the golf professionals and their caddies while serving in my volunteer capacity. I will insure that the JDC is a positive experience for all patrons by displaying knowledge about the course layout and location of amenities. I will not possess or drink alcohol or smoke tobacco products while serving as a volunteer and if I am not serving in my volunteer capacity while drinking or smoking, I will remove tournament credentials and identification. I will refrain from the use of profanity in the presence of golf professionals, volunteers and patrons. I will refrain from any form of personal abuse towards others, including inappropriate or unwanted sexual advances, and/or other verbal or physical conduct of a sexual nature or such conduct that has the purpose or effect of substantially interfering with an individual’s work and/or volunteer performance or creating an intimidating, hostile, or offensive environment. I understand that the JDC does not tolerate any form of discrimination or harassment of or by any of its employees or volunteers on the basis of race, color, religion, national origin, sex, age, disability, gender identity and/or expression, veteran status or any other protected status and I agree to refrain from any form of discrimination or harassment. I will complete all of my volunteer assignments as requested by the JDC and my committee chair(s). I understand that violations of the Code of Conduct may result in the JDC’s right to suspend or revoke my volunteer status for the current tournament and any future tournaments. Print your name:________________________________ Date:_________________________ Your signature:_________________________________ 2014 John Deere Classic Volunteer Waiver & Release of Liability In consideration of my desire to serve as a volunteer for the Quad Cities Golf Classic Charitable Foundation a/k/a the John Deere Classic (JDC), I assume all responsibility for any and all risk of property damage or bodily injury that I may suffer or sustain while participating in any volunteer work with the JDC. I understand that the scope of my relationship with the JDC is limited to a volunteer position and I am responsible for my own insurance coverage in the event of personal bodily injury or property damage which occurs as a result of my volunteer services to the JDC. I, for myself and my family members, heirs, executors, administrators and assigns, release and forever discharge and hold harmless the JDC and its officers, directors, employees, agents and volunteers from any and all liability, claims, and demands of whatever kind or nature, foreseen or unforeseen, which arise or may arise from the volunteer services I provide to the JDC. I understand and acknowledge that this Waiver and Release of Liability forever discharges the JDC from any liability or claim that I may have against the JDC with respect to bodily injury, illness, death or property damage that may result from the volunteer services I provide to the JDC or occurring while I am providing volunteer services, including but not limited to, ordinary negligence on behalf of the JDC. Further I expressly agree that this Waiver and Release of Liability agreement is intended to be as broad and inclusive as permitted by the State of Illinois, that and that if any portion thereof is held invalid, it is agreed that the balance shall, notwithstanding, continue in full legal force and effect. I acknowledge that this Waiver and Release of Liability agreement shall be governed in accordance with the laws of the State of Illinois. I grant and convey the JDC all right and interest in any all photographs or images made by the JDC in connection with my providing volunteer services to the JDC.
