 Updated as of May 27, 2011 Instructions to new LSI lab workers regarding LSI Key and Key card Access In order to obtain a key and key card to an LSI space, following is required. 1. A request for a key and key card for a new member of the lab (Undergraduate summer, directed studies and co-­‐op students, Graduate students, post docs, staff) must come from the Principal Investigator (PI) or Lab Manager by using the attached LSI Access Request form (attachment #1). 2. The request must be sent to the departmental staff that is responsible for coordinating such requests. For departments other than Cellular & Physiological Sciences, Biochemistry & Molecular Biology, Microbiology and Immunology and Centre for Blood Research, lab managers or PIs must email Sharda Muni at [email protected]. 3. The department staff or Sharda Muni will assign a Safety login for the new member of the lab. This login must be used to sign in and complete the Online Safety Orientation. 4. The department staff or Sharda will get an email notification once the Safety Orientation has been completed. At this point of time the PI or Lab manager must fill out the LSC Safety Orientation and Training Checklist (attachment #2) which must be signed by the PI. A copy of the signed checklist must be handed over to the department staff or Sharda. highlighted in attachment #2. In the instant the new member has not completed all or part of the safety courses, temporary card access will be provided until all training requirements have been completed. a. Temporary access of 1 month for short term access requests (less than 3 months) to complete the MANDATORY Introduction to Laboratory Safety Course (on-­‐line) b. Temporary access of 3 months for longer term access (over 3 months) to complete the MANDATORY UBC Chemical Safety Course, UBC Biological Safety Course and other required courses. 5. Based on the above findings, LSI staff will then authorize the key form and also order the key card from Secure Access. 6. For new term access requests (less than 3 months) LSI will assign a temporary card that is available on site against a refundable deposit of $30 (cash only). Loss of a visitor card must be reported immediately. The deposit will be refunded when the card is returned to Sharda Muni at the end of the term. It will not be refunded if the card has been lost/misplaced. 7. For access for new hires or for student/staff/pdf with longer term access (more than 3 months) LSI will order a key card through Secure Access which can be picked up from the Parking and Key Desk in room 205 on 2nd floor of General Services Administration Building, 2075 Wesbrook Mall. Parking and Key Desk requires a refundable deposit of $20 against cards and $15 against keys. The deposit is refunded when the keycard and keys are no longer needed and returned back to the Parking desk. However it is non refundable in the event the keys and cards are lost. Loss of keys and card should be reported immediately to [email protected] who will arrange for the de-­‐
activation of the key card and will also process a replacement card. However cost of replacement card will have to be borne by the user ($30) and will not be paid for by LSI. User must pay $30 (cash only) to Sharda before a signed key form for a new key card can be obtained. 8. All keys and cards must be returned to the Parking and Key desk or to LSI (depending on where you picked it up from Ȃ Parking desk or Sharda Muni at LSI) at the end of the term.
