It must show the layout of the parking lots, entrance and exit of the premises and with company stamp on it) Please attach photos showing the location of the parking lots. Eg, 10 years from 15 July 2009) 14) 15) Documentary Proof of Property Ownership If you are not the owner of the property, please provide documentary proof of property ownership. You may obtain the Property Ownership Information from the Singapore Land Authority – INLIS website at www.inlis.gov.sg. If the property is leased from a government agency, please obtain permission from the landowner in the form of an email or letter. Name of Authorised Person: ____________________________________ Designation: _______________________ NRIC No: ____________________________________ Date: ____________________________________ Signature HVP1 / 6 Nov 2014 Company Stamp Page 2 LTA/HVP1 Application Form NOTES TO APPLICANT Please read the following instructions before completing the application form. i) The applicant shall be deemed to have read and understood the Parking Places (Licensing and Control of Private Parking Places for Heavy Vehicles) Rules. ii) For new applicant you are strongly encouraged to go to www.onemotoring.com.sg website to submit your application online (Go to eServices > e-Transactions > Apply for New Heavy Vehicle Park Licence.). iii) Approval of application is subject to site inspection. iv) The license fee is $1 per heavy vehicle lot per month. Each VPC costs $6. There is no GST for licence and VPC fees. Please do not attach any payment until you receive a notification to pay when your application has been approved. v) Heavy vehicles that fall under the VPC Scheme are: a) heavy goods vehicles and concrete mixers exceeding 5000 kg in maximum laden weight (MLW) b) buses with seating capacity exceeding 15 passengers c) trailers, including container trailers, low loaders or flat bed trailers exceeding 5000 kg in MLW d) mobile cranes and recovery vehicles exceeding 2500 kg in unladen weight (ULW) vi) The licensee shall not use or permit to be used any part of his private parking place for any purpose other than for the parking or housing of heavy vehicles. Enforcement actions will be taken against licensee who fails to comply with the law. vii) For hardcopy submission, please submit the completed forms and accompanying documents to the LTA Vehicle Parking Counters at 10 Sin Ming Drive, VTL Building, Singapore 575701.
