These teams complete full assessments of Service Users who need on going services (this includes people who are
not suitable for Reablement, or for those who require on going support after Reablement has finished). Brokers
within the teams help Service Users to create a support plan using their personal budgets.
They also complete reassessments of existing clients to ensure services continue to meet needs.
They help people with complex and unstable needs through a care management approach (on-going social worker
input).
The teams also complete safeguarding investigations for people who appear to be at risk of harm.

Team Leader
These teams receive all new referrals for people in the community and in hospital.
They provide a telephone duty service and undertake emergency assessments in order to keep people safe over a 72
hour period. They will provide advice, signposting and arrange low level services such as minor equipment.
This service refers appropriate people into the Reablement service where it appears they may be eligible for services.
Some people are referred directly to locality teams for full assessment, where Reablement would not be suitable.

These teams provide care management support for Service Users using our Reablement services and by
providing OT assessments. They work with care staff in the provider Reablement team (see care services)
and complete a full assessment for discharge form the scheme. They help people to develop a support plan
for on-going services using a personal budget.
The OT team assesses people who require equipment and adaptations. They work closely with colleagues
in Housing.

To provide the population of Leicester with an integrated, efficient and responsive
Independent Living Service, which will facilitate timely hospital discharge, avoid
admissions and readmissions into both hospital and residential care, and cost effectively
enable the citizens of Leicester to live their lives as independently as possible at a place
which best meets their needs.
The service comprises of both community and bed based provision with an ethos of
maximising independence along with the use of equipment and assistive technology to aid
independent living.
Progress recommendations for major adaptations, adapted re-housing and minor
adaptations, as well as arranging the servicing and maintenance of lifts and ceiling track
hoists installed in peoples own homes.
