The diabetic educators from OSFMG Diabetic and Endocrinology office have established
recommended teaching resources for patients with diabetes to replace the materials that were
previously utilized from the OSF Healthcare System affiliation with the Joslin Diabetes Center.
The folders that were created with the Joslin Logo and the Diabetes Survival kit are no longer
available. Each unit is encouraged to review the needs of their diabetic patients and plan to provide
materials as indicated by the patient’s individual learning needs. It is suggested that units order OSF
Logo folders and then place the patient education materials in the folder.
All copies of the materials are to be ordered through People Soft or SmartWorks as indicated
in the table as these documents will be branded with OSF logo; these materials are not to be
reprinted on the individual units to preserve the document integrity and presentation. EPIC
related materials such as Exit Care and MD consult may be printed on site as printing them
through EPIC will brand them with OSF information.
The OSFMG Diabetic and Endocrinology Clinic is certified as a diabetic resource center through the
American Diabetes Association as are 5 other clinics in Bloomington, Galesburg, Pontiac, Monmouth and
Rockford. These clinics will continue to receive referrals to assist patients in managing their diabetes
care and to provide diabetic classes. The resources that these clinics will use for their individual and
class education are also listed below.
Lena Kieser, RN, has offered to be available if you have questions regarding diabetic teaching
materials; she is available at [email protected]. Per OSF Patient Education Policy,
all patient education materials provided must be provided through OSF approved resources. The
following documents from the American Diabetes Association are an approved resource. If there is a
need for education material other than those available through OSF resources, Please contact
Peggy Jacques, RN, Education Coordinator at [email protected]
Please note that this education plan is an initial way to provide our OSF patients with quality
education materials. However, we will continue to explore other ways to empower and engage our
patients on an ongoing basis. Please let Peggy Jacques or Lena Kieser know of materials that you
become familiar with so that we can continually improve this education plan.

the patient’s listed diagnosis to apply to the AVS as a measure of meaningful use patient education. Please
review the list of suggestions and select those that are applicable to the patient.
