Toelichting
Met dit formulier kunt u een Continuous Synopsis Record (CSR)
aanvragen, laten wijzigen of een duplicaat aanvragen.
De (gewijzigde) gegevens moeten ook op het "Form 3 Registration of
the amendments to the CSR" ingevuld worden.
Als u een duplicaat CSR aanvraagt, moet u bij vraag 3.1 het
certificaatnummer invullen. Dit certificaatnummer kunt u terugvinden
in de rechter bovenhoek van het CSR. Indien het certificaat ontbreekt,
vul dan het ontbrekende 'DOCUMENT NUMBER' van het CSR in.
Onvolledig of onjuist ingevulde formulieren (inclusief het niet
meesturen van de gevraagde bijlagen) worden niet in behandeling
genomen.
Vul bij elke aanvraag ook "Form 2" in (zie pagina 3).
