Background: A myocardial infarction (MI) has a profound psychosocial impact. Only few
studies focus on the prevalence of ‘return to work’ after MI. Moreover, little is known about
the employee perspective. This research aimed to study (1) the frequency of ‘return to work’
in a Dutch cohort and (2) employee perspectives on recovery and ‘return to work’ including
the role of the relevant actors after an MI.
Method: For a quantitative survey on work situation, social situation and type D personality (a
joint tendency towards negative affectivity (NA) and social inhibition (SI)) the 2005-2006
cohort was included of MI-survivors admitted to the Maastricht University Hospital and aged
39-60 years (n=41; response 69%). In-depth interviews with ten patients about their
experiences and interactions with the actors involved were held and analysed qualitatively.
Results: Up to 80% of the employees had (partially) returned three months after the MI.
Return to work was positively related to higher occupational status, leisure activities, the
cardiologist discussing return to work (0.05<p<0.10) and involvement of social insurance
officers (p<0.05). Return to work was negatively related to MI severity (p<0.05), age and the
separate assessment scales NA and SI of type D personality (0.05<p<0.10). The qualitative
study revealed that the hospital period was characterised by relief and satisfaction with the
professional treatment. Patients complained about lack of information and attention to their
experiences in daily life after discharge. Support from the home front was indispensable. The
employees experienced little support regarding return to work.
Conclusions: Although work is often resumed after MI, cardiologists seem to play an
important role regarding return to work. The patients missed however (1) a personal approach,
(2) information and (3) support while resuming work. It seems useful to work out the
responsibilities of the different actors after discharge.
