Please check that it is correct and amend as necessary. We don’t need you middle name in full, just your middle initial. Section 3 You need to decide for how long you require a postal vote and mark the applicable box. Most postal votes are until further notice and can be cancelled by the elector. Section 4 If you will be away from home, then you can choose to have your ballot papers sent to an address other than the one shown in Section 1. If you choose for your ballot paper to be sent to a different address other than the address you are registered at you must give a reason for the alternative address. The postal vote must be completed by you, no one else can vote on your behalf. If you wish to appoint a proxy to vote on your behalf please contact the Electoral Service Office. Section 5 You must enter your date of birth in the format DD/MM/YYYY and sign the form within the grey box. Try to keep the entries inside the boxes. If possible use black ink. Failure to do so may invalidate your application. Your personal identifiers will be stored securely. Each time you vote by post, your signature and date of birth provided on the postal voting statement will be checked against these identifiers, to ensure your postal vote remains secure. If you are unable to provide a signature, or you are unable to sign in a consistent or distinctive way because of any disability, or you are unable to read or write, the registration officer, in these circumstances, may grant you an exemption, which will mean you will not be required to provide a signature.
