1. Applications for a public display permit shall include the name, address, and telephone number of the applicant.

2. Applications for a public display permit shall include the location or alternate location, date, time, and the
alternate date and time of the display. IFC® 2009 (3308.2.1) and 28 TAC §§ 34.813
3. The operator or supplier shall maintain any federal or state permit(s) or license(s) required to possess and use
fireworks and shall include the name and license number of the pyrotechnic operator who is to supervise the
display. NFPA 1123 - 2006 (10.3.3), 28 §§ TAC 34.809 and 28 TAC §§ 34.813
4. The operator, supplier, or sponsor shall demonstrate financial responsibility for the fireworks display to the AHJ
by providing proof of insurance or by other approved means.

5. A site plan shall be submitted to the AHJ within a time period required by the AHJ prior to the display.

6. The site plan shall include the dimensions of the discharge site, spectator viewing area, parking areas, and the
fallout area and the associated separation distances. NFPA 1123 - 2006 (5.1.2.2)
7. The site plan shall also include the location of buildings, highways, overhead obstructions and utilities.

8. Applications for a public display permit shall also include a diagram of the grounds on which the display is to be
held indicating the:
(A) area meeting appropriate distance regulations which outlines the parameters within which the particular site
may be adjusted on the day of the proposed display to address safety concerns or wind changes from which
fireworks will be discharged; and
(B) method of spectator restraint. 28 TAC §§ 34.813

9. Applications for a public display permit shall include the name and license number of the manufacturer or
distributor licensed in Texas who is to supply the fireworks. 28 TAC §§ 34.813
10. Applications for a public display permit shall include the size and estimated number of fireworks to be
discharged, the number of set pieces, and other items. 28 TAC §§ 34.813
11. Applications for a public display permit shall include the manner and place of storage of such fireworks prior to
and during the display. 28 TAC §§ 34.813
