45 Day Procedure
Monitored by: Center Director, Family Advocates and the Management Team
The results of the Developmental Screeners and parent training outcomes will be documented in the Child Plus
and ASQ management systems.
The results of the Developmental Screenings and individual developmental plans will be shared with the
parent/guardian after screenings are completed.
Upon child’s entry into the program the Center Director will initiate the use of the 45 day tracking sheet.
For children who start after the “First Day of School”, the Center Director will ensure to initiate an individual
45 days tracking sheet and follow the first 45 days procedure.

Family Needs Assessment (LSP) - 50 % of caseload complete
1. The first Parent –Teacher Conference must be conducted prior to the first day of school, but no later than
the first 30 days.
2. Pedestrian Safety lessons must be documented on the lesson plans during first 30 days of child’s entry into
the center and presented to all new parents at the first Parent Committee Meeting or Individual Meeting
with a parent.
3. Oral Hygiene lessons must be documented on the lesson plans during first 90 days of child’s entry into the
center and presented to all new parents at the first Parent Committee Meeting or Individual Meeting with a
parent.
4. First Parent Committee Meetings will be scheduled during the month of September at all sites.
a) Sites with a capacity greater than 20 children will elect two Policy Council members and two alternates.
b) Sites with a capacity less than 20 children will elect one Policy Council member and one alternate.

Initial Developmental Screenings:
6. Ages and Stages Questionnaire, ASQ-3 Timeline
Parent completes the ASQ-3 form during the first visit with the assistance of the teacher.
Teacher reviews and scores the developmental ASQ-3 screening tool. The teacher submits the ASQ-3 to the
Center Director for review by the end of the business day on the day of the conference.
The Center Director reviews all ASQ-3’s.. If any of the children fail the ASQ-3, the Center Director will email
the child’s initials, data management system ID an d the domain(s) in which the child failed to the Education
Team members.
If child’s score is in a shaded area, and parents and teacher have a concern in thye same area of child’s
development, the center Director will email the child’s initials, data management system ID an d the domain(s)
in which the child failed to the Education Team members.
The Education Team member will observe the child to determine if an immediate referral for further evaluation
is needed. The Education Team member will work closely with teachers to develop an education plan, monitor
the implementation of it and child’s progress.
The Management Team will review all developmental screeners after the 45 days deadline.
The results will be entered into the data management system of each corresponding child. All Developmental
screenings will be filed in child’s file under the screenings section. An education plan for each child with a low
score, written by the Education Team will be discussed with a teacher and filed in the education notebook.
Teachers should review and use the education plan for lesson planning and individualization.
All follow ups and referrals will be initiated within 90 days to the local ECI provider or the local school district
if the child is age 30 months or older, or to the local School District.
The Education Team will monitor child’s progress on an on-going basis and provide support to the center staff.
7. Ages and Stages Questionnaire: Socio-Emotional, ASQ-SE Timeline
Parent completes the ASQ-SE form during the initial center visit with the assistance of the teacher.
The teacher submits the ASQ-SE to the Center Director for review by the end of the business day on the day of
the conference.
Center Director reviews the ASQ-SE for each child within 3 days after the conference has been completed and
emails the Education Team members the child’s initials and the data management system ID of any child with a
high score on the ASQ-SE.
The Education Team will observe high scoring children within 10 business days of the Center Director’s email
and determine child (ren)’s needs.

Behavior/Classroom Strategy Plans are written and monitored by the Education Team. Teachers will review
and implement written plans with the assistance Education Team.
Disability/Education Specialist will initiate the referral process for further child evaluation as needed.
All follow ups and referrals to the local ECI provider or the local School District if the child is age 30 months or
older will be initiated within and not later than 90 days from child’s entry into the program.
The Disability and Mental Health Coordinator & Disability/Education Specialist will monitor child’s progress
on an on-going basis and provide support to the center staff.

Vision and Hearing Screenings
EHS Hearing and Vision Questionnaire Timeline
EHS Family Advocate completes the Vision and Hearing Questionnaires with a parent during initial
enrollment of a child into the program and annually thereafter at “re-enrollment.”
Family Advocate places the screenings in the data management system file for the results to be entered into the
Child Plus management system and questioners filed in Child’s File.
Head Start Administrative Assistant (Health) visits centers weekly to enter normal screenings results into the
data management system and file screening results in Child’s File. The Head Start Administrative Assistant
(Health) wills scan the abnormal screening results and bring to the Nutrition and Health Services Coordinator,
who will review and enter abnormal results and the follow up plan into data management system, and provides
recommendations.
The Family Advocate shares the recommendations with the Center Director and the Parent and, in partnership
with the parent, will follow-up on any recommendations made by the Nutrition and Health Services Coordinator.
b) HS Hearing and Vision Screening Timeline
Staff certified in Hearing completes the Hearing screening within 45 days of child’s enrollment with the child
and then completes signs and dates the Hearing form.
Center Director files screenings in the data management system file.
Head Start Administrative Assistant (Health) visits center weekly to enter normal screenings results into the data
management system and files screening results in child’s file. Head Start Administrative Assistant (Health) will
scan abnormal screening results and deliver to the Health and Nutrition Services Coordinator, who will review
and enter abnormal results into the data management system, and make recommendations.
The Center Director will follow-up on any recommendations made by the Nutrition and Health Services
Coordinator.
Staff certified in Vision completes the Vision screening within 45 days of child’s enrollment with the child and
completes signs and dates the Vision form.
Center Director files screenings in the data management system file.

Heights & Weights Timeline
Center Staff weigh and measures all children age two or older within first 45 of the child’s entry into the
program and records the information on the Heights & Weights classroom chart. Center Staff gives the
completed Heights & Weights classroom chart to the Center Director.
Center Director makes a copy of the Heights & Weights classroom chart for the center and forwards original to
the Nutrition and Health Services Specialist.
The Nutrition and Health Services Specialist enters the information into the data management system which
calculates the child’s BMI.
At the end of the 45 days the Management Team meets with Center Staff to discuss every child’s screening
results and complete report cards to share results of the assessment with parents/guardians.
