Academic Planning Form (APF) Instructions
Forms due by: March 1 for summer, fall and year programs
October 24 for spring programs
1) Fill out the table below with the remaining requirements for the completion of your degree prior to your EAP program.
2) Fill out the APF. Select EAP classes that best reflect what you intend to take while abroad using the following resources:
a) The Program Courses & Credits tab on the UCEAP website.
b) The website for your EAP host university and the MyEAP Course Catalog, both accessed through the UCEAP Program
Courses & Credits web page. The MyEAP Course Catalog only shows courses taken by EAP students (from all UC campuses)
over the past five years. These courses may not be offered when you are abroad and do not reflect UCSB department acceptance.
c) Your department’s advising page found on the UCSB EAP website under Academics. Some UCSB departments (Econ,
Comm, Math, Psych) have links here to pre-approved EAP courses.
d) Advice and tutorials on how to complete this form on the UCSB EAP website under Academics.
3) Make an appointment to meet with the undergraduate advisor for your major(s) to discuss your proposed course selection
and how it will integrate with your plan for completing your major.
4) Sign the APF with your undergraduate advisor and submit it to the EAP office with your application or by the due date above.
You may need to schedule appointments with advisors so plan ahead.

Double major?
 Meet with and get signatures from BOTH major advisors.
Minor?  Planning to complete minor requirements abroad? Meet and get a signature from your minor advisor.
Undeclared?
 Complete this form with the undergraduate advisor of your intended major.
Graduating Senior? (And any student who is likely to exceed 200 units)
 Have this form signed by your College advisor AND your major advisor.
Summer Only Abroad?
 If intending to take courses for your major, get the APF signed by your advisor. If you will not be graduating abroad and do not intend to take courses for your major, you must turn in this form but you DO
NOT NEED IT SIGNED BY YOUR ACADEMIC ADVISORS.
