Sugges'ons/Thoughts —  Incorporate daily “huddles” (counts for PCMH certification) —  Protocol for follow up of ED/UC visits and hospitalizations (also PCMH) —  Protocol for follow up of missed appointments (again, PCMH) —  Develop a Spirometry strategy and goals to increase the number of eligiblepatients receiving spirometry in your office AAP CQN3 90 Day Goals July to September GOAL MEASURE CHALLENGES 100% of practices participate in monthly calls Participate in an Organized Quality Improvement Effort Busy practices and busy time of year 100% of practices enter practice level data Measure Performance and Share Data Registry speed 90% of practices achieve optimal Measure Performance and Share care Data n/a 80% of practices complete a PDSA on population management (flu vaccine, controller meds, etc) to asthma care LS3 is on September 20 – practices will have a draft of the PDSA cycle. Implementation by September 30 may be varied. Sustainable & Accountable Leadership Focused on Health Outcomes Beyond 90 Day Goals —  Update asthma population/billing query December 13-­‐ January 14 —  75% of patients uploaded have encounter form level data by the end of the collaborative Action
