New project to support nurses
to address Cardiovascular
Disease
TML is pleased to launch the Advancing Nurse Clinics in
Primary Care project. Funding and support for this project are
being provided by the Australian Primary Health Nurses
Association (APNA).
Earlier this year Tasmania Medicare Local (TML) was successful in its application for funding from the
Australian Medicare Local Alliance (AMLA) to undertake a project to support nurses in general practice to
establish nurse clinics in Tasmania. The funding for this project was withdrawn following the closure of
AMLA, however the funding has now been redirected through APNA. TML has been approached to resume
activities to undertake this project.
Through this project TML will focus on Cardiovascular Disease (CVD) as a priority health need and concern
within Tasmania.
The aims of this project are to:



Provide education to all practice nurses on the identification of people at risk of developing CVD as well
as the management of those with known CVD
Provide resources to support the identification and management of CVD
Support a small number of practices to establish CVD nurse clinics, and to evaluate these.
The attached Expression of Interest (EOI) has been distributed to practices throughout Tasmania. Given the
short timeframe for this project, completed EOIs must be returned to TML within one week. All interested
practices will then be contacted. Successful practices will receive:







resources and support to identify those at risk of CVD using the Absolute Risk Calculator and support to
implement lifestyle changes for these patients
resources and support to better manage patients with known CVD
support to develop a business model to sustain the CVD clinic
tools to measure the effectiveness of these clinics
education on identifying CVD risk and chronic disease management,
limited funds may be available to support your practice in relation to this project as needs require
a TML Program Officer and nurse mentor will support your practice throughout the project.

What is the
background of
this project?
Cardiovascular Disease (CVD) is Tasmania’s leading cause of mortality, with a 36%
higher incidence than the national average. Through application to the Department of
Health, Tasmania Medicare Local (TML) has been successful in receiving funding to
establish nurse clinics, with TML choosing cardiovascular disease as the focus for these
clinics. This funding and subsequent projects are being coordinated by the Australian
Primary Health Nurses Association (APNA). In collaboration with the Heart Foundation
(Tasmania), TML will be supporting practices to establish nurse clinics to identify people
at risk of developing CVD, as well as better manage those with known CVD.
What’s in it for
our practice?
Participating practices will be provided with resources and support to identify those at
risk of CVD using the Absolute Risk Calculator. Practices will be supported to implement
lifestyle changes for these patients, and implement good chronic disease management
processes based on a sound business model. Tools will also be provided to measure
the effectiveness of these clinics. A TML Program Officer will support your practice
throughout the project.
Free education on identifying CVD risk and chronic disease management will also be
held in each region of the state.
Limited funds may be available to support your practice in relation to this project as
needs require. Please provide details of any areas in which your practice may need
financial assistance on the attached Expression of Interest form.
The project will be completed by June 2015.
Criteria for
practices
What next?

Experience in conducting nurse led clinics, and systems in place to support nurse
led clinics.

Experience/Use of PENCAT within practice to identify patient groups.

Practice maintains quality patient data/records

Commitment to improving the identification and management of patients with
Cardiovascular disease.

A whole of general practice team approach will be required to ensure a successful
nurse clinic.
