you make will be in effect from July 1, 2014—June 30, 2015.

adoption, marriage, divorce, etc).

Easter Seals is proud to have two dedicated wellness coaches, Christy Nault and Denise Pavlides. Both are trained and ready to
assist staff in all areas of wellness, nutrition, weight loss and fitness plans, just to name a few. It is our goal to assist staff in finding their wellness balance. In addition to ongoing wellness activities, such as nutrition education, weight loss programs and exercise programs, both Christy and Denise are available to offer staff confidential coaching consultations.

If you need counseling for mental health and substance abuse, child care/ elder care, financial worries,
legal matters, relationship conflicts and more, Coastline’s trained professionals provide 24/7 counseling. These services are
provided to you and your dependents at no cost and are completely confidential. For more information please call the EAP at

Offering Critical Illness, Cancer, Accident & Hospital Policies through payroll deductions. Please contact David
Healey, Aflac Representative to inquire and enroll: [email protected]. To cancel your current
policy: contact AFLAC at 1-800-99-AFLAC and the HR Help Desk to stop payroll deductions.
