STATE OF ISRAEL – MINISTRY OF DEFENSE DEFENSE EXPORT CONTROL AGENCY (DECA) Instruction for Form 8A: End Use/User Certificate Page 1 of 2 Date of Publish: 15/05/2014 1. General a. Any exporter wishing to forward DECA (Defense Export Control Agency in the Israeli MOD) a request for an export license is required to attach an End Use / End User Certificate (EUC) form. That form shall constitute an integral part of the initial application for an export license and upon completion shall become a factor in considering approval of the export license. b. End use declaration signed by the foreign client is required in cases such stipulated provision appears in the marketing license, or in cases DECA notifies the exporter about such demand. c. Declaration form shall be filled completely with all paragraphs meticulously addressed. Writing shall be in printed letters only. d. In accordance with par. 2 (e) of the Defense Export Control Order (Licenses) 2008, the statement will be submitted in English. A statement submitted in another language requires an attached notarized translation either in Hebrew or in English. e. The foreign client may choose whether to use DECA's original form as introduced in DECA's website or have its content copied in full precision onto a formal form of his own having no IMOD or DECA titles. f. In case an application for an export license is submitted with EUC lacking details or proper signature, the whole application shall be returned without action. 2. Filling instructions a. Par.1: Name of Israeli supplier of the equipment for which the EUC is provided must be filled. b. Par.2: Name of country in which the ultimate user exists must be filled. c. Par.3: Name of ultimate user of the equipment must be filled. d. Par.4: Contract or purchase order's identification details as signed between the foreign client and the Israeli supplier must be filled. e. Par.6: Expected use of the equipment for which the EUC is provided must be detailed. In case equipment is actually components, systems in which those items are to be integrated must be specified. g. Par.7: Name, quantity and description of the equipment / items must be filled. h. Par.8: A statement to be filled by the interim agent/ interim user who directly the equipment / items / knowhow / service that is destined for another ultimate end user. i.
