B ria n Lam m , Assis ta n t P ri nc ip a l MATERIALS PICK -UP AUGUST 5TH With school about a month away, it is time to start thinking about the 2014-2015 school year. On Tuesday August 5th from 8:00 AM to 7:00 PM students and parents need to pick up books and pay fees. Class schedules will also be available at that time. Important Dates • August 5th - Materials Pick -Up, 8:00AM - 7:00 PM • August 18th - Meet the Teacher Night, 5:30 PM7:00 PM • August 19th - First Day of School, 7:30-2:35 • August 20th - Wednesday Schedule, Late Start, 7:502:35 • August 20th - School Pictures. Students may wear a school appropriate nonuniform shirt for picture day. However, uniform bottoms must be worn. • August 27th - Wednesday Schedule, Late Start, 7:502:35 If you need to apply for free or reduced lunch, please go to the FMS cafeteria first. If you have received the paperwork that you are direct certified, please go to the conference center. Included in this envelope, please find music department information, a parent/ student compliance and release form, a health form and an electronic network access form. Please complete this paperwork and bring it with you on August 5th. At Materials Pick-Up, Grades 6-8 will receive textbooks and get their locker assignments. Students are to take their textbooks to their locker after receiving them. 5th grade students will receive textbooks and lockers during the first few days of school. In order to pick up textbooks, students will need their schedule and textbook receipt (or free book waiver). After picking up books, it is very important for students to put their name in them in ink. There will also be a sports table with information regarding fall sports in the FMS conference center. Students must have a current sports physical before they can tryout or practice. Sports physicals are good for 395 days. PBIS CONTINUES THIS SCHOOL YEAR Freeport Middle School will continue implementing Positive Behavior Interventions and Supports for the 20142015 school year. Positive Behavior Interventions and Supports (PBIS) is a proactive systems approach to establishing the behavioral supports and social culture needed for all students in a school to achieve social, emotional, and academic success. As a Response to Intervention model, PBIS applies a three-tiered system of support, and a problem-solving process to enhance the ca- pacity of schools to effectively educate all students. The PBIS Network builds capacity of schools, families, and communities to promote social and academic success of all students, including those with emotional/ behavioral and other disabilities. Key focus areas of the Illinois PBIS Network include: Prevention-based school -wide systems of positive behavior support Data-based decisionmaking for instruction of behavior and academics UNIFORMITY OF DRESS Wraparound planning for students with complex emotional and behavioral needs and their families Community-based supports for families, youth and schools We would love to have parents and community members join our team. For more information about PBIS, visit http://www.pbisillinois.org/ SFCP If you are interested in being a part of our PBIS team, please call FMS at 232-0500 For the 2014 - 2015 school year, FMS students in grades 5-8 will continue to follow the uniformity of dress policy. Middle school students are able to wear the common shirt colors of black, white, light blue, navy blue, grey and orange. Colors for bottoms (pants, shorts, skirts, skorts, capris, etc.) must be navy, black or khaki and CAN NOT BE DENIM. Also, NO JEGGINGS are allowed as they are considered spandex. Small brand logos no larger than three inches may be worn on collared shirts, sweaters, sweatshirts, and fleece. Uniform items may also be purchased at a variety of local retailers. MEAL PRICES AND SCHOOL ID’S Breakfast prices for the 2014-15 school year will be $1.60. Lunch prices will be $2.40 for the regular lunch program which includes the salad bar. Ala carte items are also available for an additional cost. All students will have an account number (their student ID number) that is used when purchasing lunches. Parents may choose to put money on the student’s account in advance to avoid carry- ing money every day. Students can use their ID from the 13-14 school year until pictures are taken. FSD 145 school policy requires students to wear their ID’s at school (ONE ID and lanyard will be provided). Replacement ID’s and lanyards will be available for $5.00 (ID) and $2.00 (lanyard). Students are not to decorate or obscure the information on their ID’s, or they will be required to purchase a replacement. SPORTS AND ACTIVITES “In addition to our award winning music program, we offer a wide range of sports and activities for our students.“ There is something for everyone at FMS when it comes to getting involved in your school. In addition to our award winning music program, we offer a wide range of sports and activities for our students. Beginning with fall semester, we offer volleyball, football, soccer, and cross country running. Students involved in these sports compete with other schools across the Northern Illinois area. Most practices for these sports will begin prior to the beginning of the school year, so look for the sign-up table at FMS Materials Pick-Up for information regarding when practices begin. Students must have a current sports physical prior to trying out for any sport. For sports held at Carl Sandburg Middle School, an activity bus for student athletes will be provided to get them to practice. However, athletes will be responsible for arranging their own ride home. Student athletes need to be aware that they must pass eligibility requirements in order to participate. Remember, no pass, no play! Other activities starting up in the fall include Builder’s Club, Student Council, Yearbook and Peer Mediators. Speech and Quiz Bowl are both IESA (Illinois Elementary School Association) activities, and along with cross country, wrestling, chess, cheerleading and track will participate at the State competitive level. I M P O R TA N T M E D I C A L I N F O R M A T I O N Attention Future 6th Graders: Upon returning this fall for sixth grade, you will need a completed school physical form called the State of Illinois Certificate of Child Health Examination. ISBE states an exam must be completed MEET THE TEACHER within 18 months of May 15th 2015. Attention 6th-8th graders: The Illinois Department of Health requires students in grades 612 to show proof or recent pertussis vaccination (Tdap vaccine). If you have any medical questions , please call our school nurse Mrs. Buss. On August 18th, we will have Meet the Teacher night from 5:30 - 7:00 P.M. During this time, parents and students will be able to meet their team of teachers as well as their elective teachers. The 7th grade team will meet from 5:30 - 5:50 P.M. and the 8th grade team will meet from 6:30 - 6:50 P.M. in the following areas: 7th Grade - FMS Theatre 8th Grade - FMS Cafeteria
