Marcus Daly Telemedicine Network (MDTN) is a combination of 2 clinics, in the state of
Montana and Marcus Daly Hospital (HCP 10750). The Consortium was created to facilitate and
assist in the deployment of high-speed networking for rural hospitals and clinics to allow rural
patients improved access to health care services
General Project Summary
Via the MDTN, the two rural healthcare clinics would be linked to Marcus Daly Hospital
allowing them to perform telemedicine and transmit all types of medical and imaging data. With
the new connection, healthcare providers will be able to meet new requirements, treat more
patients, consult remotely with specialists while patients are at the hospital, and send and receive
all radiological and digital imaging expeditiously.
Contract Term
HCP is seeking to enter into a 36 month term contract, a 36-month term with an optional 2-year
renewal or a 60-month term with no optional renewal period so separate pricing should be
provided.
Circuit Pricing Requested
Marcus Daly Telemedicine Network is contemplating 1 to 3 different circuit speeds for each connection.
If they have the need, they may want to increase the speed of any circuit during the term of the contract to
a faster speed without signing a new contract. All Clinics are looking for connections back to HCP 10750
so they can transmit data and use the Internet service.

Redundancy and Equipment Needs
HCP is seeking redundant connectivity at HCP 10750. Due to the remote location, outages are common,
whether due to carrier maintenance, equipment outages and physical fiber damage. This interrupts SIP
telephone and hosted Electronic Health Records connectivity, and can place patients at risk. MDTN is
seeking a solution with hardware and internet services using BGP and unique geographic paths to enable
continuity in the event of an ISP failure, minimizing outage time.
Marcus Daly Telemedicine Consortium is seeking the implementation of layer-3 connections to
locations that need EHR and telephone connectivity to increase security and guarantee performance.
HCP is seeking the implementation of redundant Unified Threat Management appliances to protect from
internet threats with features such as Intrusion Prevention, Stateful Packet Inspection and Anti-Malware.
The configurations are based on Cisco Systems products. Whenever the name of a manufacturer, trade
name, brand name, or model and catalog numbers followed by the words “equivalent”" are used in the
specifications it is for the purpose of item identification and to establish standards of quality, style, and
features such information is not intended to only solicit proposals for Cisco Systems products.”
. Bids on equivalent items of the same quality are invited. However, to receive consideration, such
equivalent bids must be accompanied by sufficient descriptive literature and/or specifications to clearly
identify the item and provide for competitive evaluation.

When submitting a bid, the carrier should provide the hospital a bid document delineating all costs
associated with providing the requested service. An itemization of estimated taxes and surcharges
should be provided and for both service and equipment. The provider with the lowest cost will
receive the highest score.
2. Bandwidth
Vendor needs to be able to provide bandwidth speeds listed.
3. Quality of Transmission
Circuits with industry-standard, or better, Quality of Service (QoS). Vendors will provide a SLA
outlining the Quality of Service Metrics.
Recommended Targets:
a. Reliability (uptime) 99.9%
b. An average end to centralized Hospital site delay of less than (<) 20 milliseconds
c. Jitter <20 ms
d. Packet Loss <1%
4. RFP Compliance
Bids submitted following the RFP requests will receive the highest score
If the selected vendor’s services do not meet the above requirement then the HCP has the right to cancel
the signed agreement without a termination penalty. Vendor must have a valid SPIN issued by USAC.

PROPOSAL EVALUATION
Proposed vendor responses will be evaluated on the following criteria, using a ranking of 1 to 10, with 10
being highest score, multiplied times the weighted percent:

The selection will be based on all factors indicated in this section and the winning bid may not be the
lowest cost bid. The point scale listed above will be used to determine the most cost-effective solution for
the HCP.
SUBMISSION / QUESTION PROCESS
1. A letter signifying the bidders’ intent to bid on this project should be received via email no later than
14 days after the RFP is posted for bidding. Letter of intent emails should be sent to
[email protected] with the following information: a) name and address of the company; b) the
company’s authorized representative’s name, title, address, phone and email address. Only bidders
submitting such a notice will be eligible to send questions and receive answers concerning this RFP.
2. Questions concerning this RFP should be submitted via email to Geoffrey Boggs at
[email protected]. Vendors are invited to submit written questions and/or requests for
interpretations/consideration/acceptance concerning this RFP within 15 business days from the
posting of the RFP. All submitted questions and the applicable responses will be compiled and will
be sent to all bidders who submitted a letter of intent to bid 18 to 20 days after the RFP is posted. Any
communications not sent to the email address above will not be entertained.
3. In the event it becomes necessary to amend, add to or delete any part of the RFP, the amendment will
be posted on the USAC website.
4. Bid proposals must be sent via email to Geoffrey Boggs at [email protected] no later than
midnight Eastern Standard Time on the 28th day after the posting date as published on the USAC

If the Consortium Leader fails to sign a contract before the winning bid expires, which results in a change
in price by the winning bidder, it reserves the right to reopen the RFP for bids.
