All intending Haj Pilgrims of the State are hereby informed to collect the prescribed Haj Application Form (HAF) along-with Guidelines for Haj 1435 from the offices of respective Dy. Commissioners; except those of district Srinagar who shall obtain the forms directly from Haj House, Bemina. The application forms shall be available on all working days from 1st February, 2014 to 15th March, 2014 during office hours, free of cost. Last Date: The application forms, complete in all respects, along-with a non-refundable processing fee of Rs.300 deposited in State Bank of India on the prescribed pay-in-slip, shall be accepted from 1st February, 2014 to 15th March, 2014. All applicants of Kashmir Valley shall have to deposit their Haj Application Forms directly at Haj House, Bemina, Srinagar while as those of Jammu Division and Leh & Kargil Districts shall deposit their application forms through their respective Dy. Commissioners by or before 15th March, 2014. No application shall be entertained after this date. Guidelines: The eligibility criteria is given in the booklet containing Guidelines and Haj Application Form and must be read thoroughly. The detailed guidelines are also available on the official website of the Haj Committee of India www.hajcommittee.com. The main features are summarized below: i) Passport: All applicants shall mandatorily be in possession of Valid International Passport at the time of submission of Haj Application Form. The Passport should have been issued on or before 15/03/2014 and valid atleast upto 31/03/2015. Haj Once in a life time through Haj Committee of India: A person who has performed Haj through Haj Committee of India earlier shall not be eligible to apply for Haj 2014. This condition is waived only in case of Mehram of a female pilgrim and companion of a 70+ pilgrim, if no other Mehram / Companion except the one who has performed Haj earlier is available. However, such repeater pilgrim (mehram or companion) shall have to pay full airfare, as prescribed by the Ministry of Civil Aviation, Govt. of India. All applicants shall have to submit a solemn declaration in this regard alongwith the application form strictly on the format attached with the guidelines. iii) Reserved Category Pilgrims. Following categories of applicants shall be treated as Reserved Category Pilgrims: ii) A) B) The applicants aged 70 years or above as on 31/01/2014 (i.e. born on or before 01-02-1944). The applicants who have consecutively applied for last three years i.e. in the year 2011, 2012 and 2013, but did not get selected. The applicants under category "A" i.e. of 70 year plus age, shall have to be necessarily accompanied by a companion from among the following relations, otherwise their applications shall not be entertained: Husband / Wife, Brother / Sister, Son / Daughter, Son-in-law / Daughterin-law, Grand-son / Grand-daughter, Nephew / Niece. The category "B" persons i.e. 4th time applicants, as also the companion of 70 year plus applicants, shall not have performed Haj earlier, either through Haj Committee of India, Private Tour Operator or any other means. To this effect they shall have to submit a solemn declaration on the prescribed format given in the Haj Guidelines. The persons falling under these reserved categories shall have to deposit the original International Passport at the time of submission of Haj Application Forms in the specified offices. iv) Categories of accommodation in Makkah - al - Mukaramah: The details about accommodation types and facilities are given in the Guidelines which must be perused carefully before indicating option in the Haj Application Form. It is clarified that there are two categories of accommodation available in Makkah - al - Mukarramah. a) Green: Green Category accommodation which is situated upto 1500 metres from outer periphery of Haram-e-sharief (not from Khana-e-kaba). This category entails higher payments, which does not mean star hotel accommodation. Buildings in the Green category are likely to be old and on elevated area due to natural topography of the city of Makkah. Room size may vary for accommodating 1 to 20 persons. The pilgrims shall have to share toilets / rooms / kitchens. There is no transport facility for Green Category pilgrims. b) Azizia: Azizia Category accommodation is situated in the Azizia area of the Makkah City which is located about 7 km from Haram Sharief. Transport facility is provided to the Pilgrims staying in Azizia to and from Haram Sharief. The accommodation facilities at Madina Munawarrah and Mina are same for both categories. v) a) b) c) d) e) f) Medical Screening: As per Haj Guidelines, persons who are not mentally and physically fit to perform the pilgrimage are not eligible to apply. Accordingly, persons who are physically or mentally incapacitated or inflicted with serious ailments or communicable disease and lady aspirants in advanced stage of pregnancy are not entitled to apply for Haj. As such, there shall be two tier medical screening of the applicants. First, at the application stage when the applicant shall get himself examined and obtain Medical Screening Fitness Certificate on the reverse of Haj Application Form. Subsequently, it is proposed to put all selected pilgrims to thorough checkup and get them examined by a competent Medical Officer in order to ensure that medically unfit pilgrims do not proceed for Haj as they are not able to perform Arkan-e-Haj and become an administrative burden as well as a source of problem for the fellow pilgrims. Enclosures: The applicants shall enclose following documents with Haj Application Form in duplicate without which the application shall be treated as incomplete and subject to rejection: Proof of address. Original Passport / Copy of passport, as applicable. Cancelled blank cheque / copy of first page of bank passbook / Certificate issued from bank indicating bank account details of the applicant. Pay-in-slip indicating payment of processing fee. Solemn Declaration (s) on plain paper. Medical screening and fitness certificate duly filled in. All Intending Pilgrims are requested to go through the guidelines and fill up the application form correctly and completely after ensuring that they fulfill the prescribed eligibility criteria so that they are not put to any inconvenience later on. Any wrong information provided in the Application form or enclosed documents shall result in cancellation of the application at any stage even at the time of boarding the flight and may result in forfeiture of the entire amount deposited.
