EndoWorld® GYN 38-1 04/2012-E In-house Training Solutions from KARL STORZ In-house Training Solutions from KARL STORZ – Endoscopic Training Possibilities Directly Inside the Hospital The permanently progressing development in minimally invasive surgical techniques requires continuous education as well as adequate training of the surgeons. In laparoscopy this affects not only surgeons but also gynecologists and urologists. The elementary laparoscopic psychomotor skills that should be learnt and practiced are: • Camera navigation with 0° and 30° telescope • Evaluation of depth of a 2D picture • Hand-eye coordination • Handling of instruments without tactile feedback • Fine motor skills • Bimanual coordination Further essential skills of a surgeon are practical basic knowledge of instruments and devices in daily OR routine as well as surgical skills of a particular intervention. A particular challenge is to aquire laparoscopic suturing skills, which need to be practised intensively. The traditional apprentice-tutor education model requires a large number of surgical interventions and is very time-consuming. Studies have shown that only limited learning is possible by observing interventions. Live surgery with all its burdens is not an ideal learning environment. Additionally, it is very problematic to objectively evaluate the clinical competence of various difficult operative interventions. The pressure on cost-efficiency is continuously growing and the intervening OR time also complicates matters. In order to facilitate the establishment of a training environment where the above mentioned skills can be learned, practised and consolidated, KARL STORZ offers various training stations. Thus the required endoscopic skills can already be practised at an early stage by residents in order to qualify for employment at the OR table. KARL STORZ supports in-house training and education by special price conditions of the training stations offered. 2 3 The core piece of these training stations is the TELE PACK system, which offers a camera, a light source, a monitor and a documentation module, incorporated into a single compact and mobile unit. Various training models are available for different OR techniques (e.g. hysteroscopy and laparoscopy), and can be individually equipped with artificial or biological preparation. In addition each training station includes the essential original instruments which are required for each operative technique and which are also used in the operating theater. In case of need, further devices (e.g. an HF generator or suction-irrigation pump) can complement the training station. In the following pages you will find further details on the individually equipped training stations from KARL STORZ, which can be used interdisciplinary and continuously for internal education purposes of your hospital. “Suturing” Training Station This laparoscopic training station was especially designed for the training of suturing and knot tying skills. The training station consists of the SZABO-BERCI-SACKIER laparoscopy trainer, the TELE PACK system as well as laparoscopic instruments particular to endoscopic suturing. The laparoscopy trainer is ideal for the training of intracorporeal knot tying as well as laparoscopic suturing and therefore improves the skills in both disciplines. The TELE PACK system is a compact unit consisting of a light source, camera, monitor and documentation unit and provides the imaging during training. The instruments contained in the set consist of a 0° HOPKINS® II telescope (10 mm), two needle holders, grasping forceps as well as METZENBAUM scissors. In contrast to the classical training concepts, the demanding specific skills of the surgeon are also trained outside the operating theater by using the training model for laparoscopic surgery, the EAGS model. Although many different systems, including animal models and simulators have been proposed, an in-house structured and validated method, easily implemented anywhere, has been missing. For these demands a training model such as the one displayed above is adequate in order to train the specific skills and monitor the learning process. The implementation of the training model during Skill Evaluation Workshops organized by the European Academy of Gynecological Surgery highlights the special suitability of the trainer during further education of laparoscopic skills. The realistic conditions are achieved by the organs being made of neoderma which are inside the trainer. The abdominal wall is reproduced to the consistency of the human abdominal wall. Various laparoscopic surgical steps, especially laparoscopic suturing can be practised well with this model. For example, intracorporeal knot-tying as well as the introduction of needles through the abdominal wall can be realistically simulated. Moreover, the grasping and dissection technique with different instruments can be exercised and improved. Certain practise is also necessary for the appropriate camera navigation. The LYRA trainer provides a good possibility to practise the skills in camera navigation, for example with a 30° telescope. A uterus made of biological tissue as well as neoderma can be introduced inside the trainer. Therefore a very realistic training possibility has been created which is ideal for learning the atraumatic access into the cavum uteri without speculum or tenaculum forceps to hook the portio. This method is also called vagino-cervico hysteroscopy and is increasingly carried out without any type of anesthesia. The introduction of the hysteroscope can be simulated realistically. This offers the possibility to the surgeon to train the hand-eye coordination. With semirigid instruments, for example, biopsy sampling, septum dissection or polyp extraction can be simulated. The imaging is also accomplished via a TELE PACK system. A resectoscope is applied instead of a diagnostic or office hysteroscope. Operative hysteroscopy is very demanding and can be learnt by frequent and regular practise. During surgery, hand-eye coordination is a very complex task. This can be trained by targeted practise on the trainer. Amongst other possibilities, precise resection of pathologies can be exercised by means of different uterine models. The imaging is accomplished by the TELE PACK system which is included in the set. The training station is furthermore equipped with a HAMOU® ENDOMAT®, which dilates the cavum uteri with pressure and flow control. Moreover, it allows for active suction and thus enables continuous-flow interventions. An HF generator, which is dedicated for both unipolar and bipolar resection, is required for the application of a resectoscope.
