2014 Winter Sports Information* (as of 9/30/14)
ALL students are encouraged to participate in HBA sports regardless of skill level or experience.
We also encourage students to participate in multiple sports if possible. *Listed below is the
basic try out/practice information and coaches contact information for the fall sports. Please
note the requirements for try outs:
1. 2014-15 Athletic Handbook Agreement Form (must be printed from the school website:
www.hba.net) turned in on or prior to the first day of try outs.
2. Updated Student Health Record Form (must be picked up from school nurse and
accompany student to his/her physical exam…and be signed and dated by physician) turned in
on or prior to first day of try outs. Students who do not have physical prior to first day of try
outs must contact program coordinator to discuss the possibility of a late try out.
3. 2014-15 Pac-Five Participation Clearance Form (For Pac-Five sports only) This must be
picked up in person at the HBA Athletic Training Room from either Sam Lee or Shannon
Keen. This cannot be accessed via the school website as it contains a signature from the HBA
Athletic Department.
4. Concussion Baseline Testing for students in grades 7, 9, 11 in specific sports. These sports
are volleyball, basketball, football, baseball, soccer, wrestling, judo, cheerleading. Contact the
HBA Athletic Training Room at 564-0341 for more information.
