Region: South West Location: Machen Quarry SWales Date: 03/10/07 Headline Articulated HGV Roll Over At approximately 8.30am on the 3rd of October an morning of the Articulated HGV operated by KCS transport was delivering Smm dry stone to Machen transported from Craig yr Hesg Quarry. The Drive has been suspended from All Hanson UK sites pending the POI conclusion and the vehicle will not be back in service until it has been repaired inspected and tested. A full investigation is in progress and POI is to be scheduled. If the franchised driver had been in his cab this incident could have potentially resulted in a fatality. KCS vehicle had successfully tipped moments before the incident in the designated tipping zone without any issues. A The driver involved in the incident failed to tip in the designated tipping zone, assess the ground conditions but also failed to take into consideration the vicinity of other vehicles within the tipping hazard zone of his vehicle. The driver parked his vehicle on the toe of the stockpile with the near side wheels on the slope and the off side in a puddle on the edge of the haul road access. As a result of his failure the vehicle was not level at the point of rising the tipper body and when the load was raised only two rams approx 10 feet the vehicle went over on the franchised vehicle parked in the designated washout area. RHSA, AOM Unit Manager and the Logistics controller all attended the scene to investigate the incident. A delegation went to KCS Transport and received confirmation that the driver would be suspended pending a disciplinary hearing. Tipping Rules and Procedures Must Be Followed Without Exception!!! a Hanson Building Products Lorry Tip Over Measham, Brick Works ih I A December 2010 lorry delivering sand arrived with the load solid due to low overnight temperatures (_12°). It started to tip the load, the one side of the load discharged and the weight of the remaining load caused the articulated lorry to tip over. No personal injuries were sustained in this incident. At approximately 04:45 hrs an articulated lorry attempted to tip sand into a Storage Bay, the lorry raised the load, and the right side of the load came out, but the remaining sand did not come out as it had frozen solid. The weight remaining in the trailer caused it to tip over and discharge its load, causing substantial damage to the trailer and cab. The driver did not sustain any injuries as a result of this incident. He asked for assistance with his load has it had not been loaded correctly (it was loaded unevenly on the passenger side) a load of sand was placed in his trailer to level it. As the trailer was lifted, the load on the driver's side came out of the trailer, the weight of the remaining sand caused the trailer to tip over onto the off side of the vehicle. A contract lift is taking place to recover the vehicle. When loads that are wet, damp and have been exposed to extremely low temperatures for a long period, drivers must not be allowed to discharged until it is safe to do so. ?e safetyalert wor?healthy fifu Hanson Quarry Products Articulated Tipper Trailer Roll Over Whatley Quarry, South Region 17th A November 2010 collect customer's articulated tipper trailer rolls over after attempting to tip off overloaded material. The driver remains in the cab with his seat belt on as the tractor unit is shaken, the unit remains upright. No injuries were sustained as a result of this incident. An Ex works lorry was loaded at the dry stone lorry load out plant. The articulated tipper was under loaded by 1.5t. After check weighing on the exit bridge the tipper came back to the stock yard for a top up of material from a loading shovel. The Loading shovel driver misunderstood the driver's request via hand signal of 1.5t and proceeded to load a full bucket approximately 8t. The loading shovel operator realized that he had overloaded the tipper, he stopped loading and told the lorry driver to go down the end (tip off area) too tip off before re-Ioading. The driver proceeded towards the tip off area but as it was partially underwater due to surface rain run off he decided to tip next to the 3mm sand stock pile. Whilst tipping the driver could see the body lean over. The unevenly distributed load then forced the body of the articulated trailer over onto its side The cab remained upright, however the trailer had lifted .. the drive wheels when it rolled over, cork screwing the trailer chassis and splitting the side of the tipper body. The loading system at site is currently undergoing re-calibration to ensure more accurate loading. It was recognized in the inquiry meeting that clearer communication between the weighbridge, loading shovel operator and the tipper driver may have prevented the subsequent overload. The site intends to trial a fix radio communication point to provide drivers a means of communication with the site loading shovel operators. The tip off area has already been re-Iocated to a more suitable area with better surface drainage. . Approved recovery company Dragon Recovery contacted and vehicle and site details provided to response crew. All 13.4 control of contractor documentation applied and completed. 3. 4. Recovery crew escorted to incident scene to evaluate recovery requirements and produce a task and site specific RA ISWP approved by site management, transport department & AHSA. Site management produced RA I SWP specific for on site employees involvement in the task. 5. Recover team and designated site employees briefed and sentry set in place when excision bund 6. was removed. 7. 8. Recovery team effected a successful controlled vehicle roll which minimised the risk of injury and vehicle damage. The driver was delivering in the designated tipping point and his vehicle was fitted with an inclinometer. full investigation is in progress to establish the route and underlying causes to this event and an update will be published. No injuries were sustained, the driver was D&A tested and the vehicle was recovered safely. There was potentially a risk of HGV roll over due to the loading of materials to one side as you can see from. the photo, if the vehicle attempted to discharge its load. Articulated lorries especially bulk containers are particularly vulnerable to tip over as we have all seen in the past. What could have been the worst outcome: Major Injury, Death or damage to vehicles and plant due to HGV roll over. Action The haulier manually levelled the load before attempting to deliver this material. All load out operatives reminded that they must when loading these types of vehicles, ensure a machine with the correct reach capabilities is used and the load is placed centrally down the length of the tipper body. Load not central to tipper body. ?e safetyalerl work§healthy·;;;- ==.d "----'-'.= 002.10.12 Hanson Quarry Products Articulated Tipper Role Over Whatley Asphalt Plant 11 th October Articulated Tipper Roll Over A contracted hired haulier collected grit stone dust from Gelliegaer quarry and attempted to tip off at the delivery point at Whatley Asphalt plant. The driver recognised that his load was unevenly loaded and requested assistance from the site to counter balance his load. On the drivers request additional material was added to the tipper and when the driver attempted to tip a second time the load did not release and at full extension the tipper ram failed resulting in the tipper body rolling over. The initial investigation has established that the grit stone dust had a high moisture content, and the load was not evenly distributed. A full investigation and panel of inquiry has been scheduled and its findings will be communicated as an update to the UK business. Action: 1. Only load materials that are suitable for delivery. 2. Assess the suitability of your loading equipment in relation to the size and height of tipper bodies to ensure they can load safely. 3. Ensure every load is inspected at the loading site, never release an unstable load onto the highway. 4. Never add additional material to balance loads if it impacts on the maximum load capacity of the tipping system. This is not the first occasion that an articulated tipper has rolled over at a delivery point, it is essential that due care and attention is taken at both the loading point and tipping points. Never release an unsafe load on to the highway, always inspect the load before dispatch, always ensure the delivery tipping point is suitable, never add additional weight to an unstable . load. ?
