Chair Nelson called the May 27, 2014 meeting of the Board of Directors to order at 7:02p.m.

ACTION ON MINUTES
Chair Nelson presented the Minutes from the May 12, 2014 Regular Board Meeting and asked for additions
or corrections. Hearing none, the Board approved the Minutes as submitted.
SUPERINTENDENT’S REPORT
KHS Student Representative, Genesis Ortega
Genesis reported three KHS students received scholarships from the Airport Rotary Club.
KHS students and staff continue to prepare for graduation on Thursday, June 12.
WHS Student Representative, Ivan Garcia
Ivan reported WHS students are participating in the second “Spirit Week” of the year.
WHS students and staff continue to prepare for graduation on Friday, June 6.
Student Presentation, Clear Lake Elementary
Clear Lake Principal John Luhman introduced teacher Meegan Cotter. Ms. Cotter’s 4th grade students
recently created cards for military veterans participating in the South Willamette Valley Honor Flight to
Washington D.C.
4th grade student, Anthony Middleton read his card to the Board.
Student Presentation, Prairie Mountain School
EWEB Grant Coordinator Cathy Bechen, 8th grade teacher Kelly Leguizamon and John Femal with EWEB
spoke about the wind turbine student project.
8th grade students, Nathan Edwards and Cheyenne Cole shared highlights from their trip to Washington D.C.
on April 24 to participate in the National Kidwind Challenge.
April Financial Statement, William Lewis
Business Services Director William Lewis reviewed the April financial statement showing an estimated
Ending Fund Balance of: $8,194,393.00.

Superintendent Chris Parra explained the process used to determine alternative placements for students.
Bond Project Update
Superintendent Gill stated a pre-construction meeting at Malabon occurred earlier today. The preconstruction meeting for Fairfield is Thursday.
Excavation continues at WHS and concrete footings will be constructed later this week.

The Board directs its district health services staff to coordinate with health personnel from other public
agencies in matters pertaining to health instruction or the general health of students and employees. In
addition, health services staff participate and collaborate with district level committees as appropriate.
A. The district shall maintain a prevention oriented health services program which provides:

Emergency nursing for all students presenting with acute/urgent illness, injury, abuse, or
mental/emotional health issues, including space separated from other students adequately equipped
for providing first aid; including standing orders for emergency use of certain medication as per the
Medical Director;
Development of emergency protocols for all students with identified serious health problems;
Maintenance of pertinent health information on the students, as required by Oregon statutes and rules;
Development of nursing care plans for students with special or chronic health needs;
Availability of health counseling, resources, and instruction to student and staff individually or in a
classroom presentation;
Training of school personnel in administration of medications and responding to emergencies;
Communicable disease control including referral for diagnosis and treatment;
Assessment of student immunization and monitoring status for compliance;
Referral for diagnosis, treatment and evaluation of health concerns; and,
Health appraisals through vision screening and other screenings such as scoliosis and dental screening
as resources permit.
B. Recognizing the prevalence of severe life threatening allergies among school populations and that the
risk of accidental exposure to allergens can be reduced in the school setting. The district will work in
cooperation with parents, guardians, students, district staff and medical professionals to minimize risks
and provide a safer educational environment for all students. “Severe Allergy” is defined as a lifethreatening immune response to a specific substance (e.g., food, bee stings, pollen, latex). A Severe
Allergy Handbook will be developed to serve as a guide; it is to be updated every two years by the
District Wellness Committee.
BC. Communication and Informed Parent/Guardian Consent
The district recognizes its responsibility to notify parents/guardians in advance of any non-emergency,
invasive physical examination or screening that is required as a condition of attendance; administered and
scheduled by the school in advance; and not necessary to protect the immediate health and safety of the
students, or of other students.
Notification will be provided at least annually at the beginning of the school year or periodically
throughout the year in the school calendar, newsletters or letters and will include specific or approximate
dates during the school year when such activities are scheduled or expected to be scheduled.

Parents/guardians shall have the opportunity to request their students be exempt from participation in any
screening conducted by the school.
In order to foster communication of the student’s health care needs, parents/guardians are requested to
complete and sign a health assessment form at the beginning of each school year. Every effort will be
made to communicate with the parent/guardian regarding their student presenting a serious health care
concern to health services.

RESOLUTION: CONSENT AGENDA/PERSONNEL ACTION
The Board of Directors, School District No. 52, Lane County, approves personnel action involving licensed
employees and extra duty contracts at each regularly scheduled School Board meeting. If the Board of
Directors would like to discuss any of these recommendations in executive session, the employee should be
identified by the number preceding the name and it will be withdrawn pending further instruction from the

Directory information means those items of personally identifiable information contained in a student
education record which is not generally considered harmful or an invasion of privacy if released. The
following categories are designed as directory information. The following directory information may be
released to the public through appropriate procedures:
1. Student’s name;
2. Student’s address;
3. Student’s telephone listing;
4. Student’s electronic address;
5. Student’s photograph;
6. Date and place of birth;
7. Major field of study;
8. Participation in officially recognized sports and activities;
9. Weight and height of athletic team members;
10. Dates of attendance;
11. Degrees or awards received;
12. Most recent previous school or program attended.
Public Notice
The district will give annual public notice to parents of students in attendance and students 18 years of
age or emancipated. The notice shall identify the types of information considered to be directory
information, the district’s option to release such information and the requirement that the district must, by
law, release secondary students’ names, addresses and telephone numbers to military recruiters and/or
institutions of higher education, unless parents or eligible students request the district withhold this
information. Such notice will be given prior to release of directory information.
Exclusions
Exclusions from any or all directory categories named as directory information or release of information
to military recruiters and/or institutions of higher education must be submitted in writing to the principal
by the parent, student 18 years of age or emancipated student within 15 days of annual public notice. A
parent or student 18 years of age or an emancipated student, may not opt out of directory information to
prevent the district from disclosing or requiring a student to disclose their name, identifier, institutional

5. A list of personal characteristics that would make the student’s identity easily traceable such as

7. Other information requested by a person who the district reasonably believes knows the identity of
the student to whom the educational record relates.
Prior Consent to Release
Personally identifiable information will not be released without prior signed and dated consent of the
parent, student 18 years of age or older or emancipated.
Notice of and/or request for release of personally identifiable information shall specify the records to be
disclosed, the purpose of disclosure and the identification of person(s) to whom the disclosure is to be
made. Upon request of the parent or eligible student, the district will provide a copy of the disclosed
record.
Exceptions to Prior Consent
The district may disclose personally identifiable information without prior consent under the following
conditions:
1. To personnel within the district who have legitimate educational interests;
2. To personnel of an education service district or state regional program where the student is
enrolled or is receiving services;
3. To personnel of another school, another district, state regional program or institution of
postsecondary education where the student seeks or intends to enroll;
