February 1, 2014 Dear Applicants, Congratulations on your upcoming graduation. You should be very proud of your accomplishment. I am sure you can remember your excitement when you began high school four years ago, and now you are once again beginning a first year. I am very excited for you all, and I wish each of you tremendous success. In order to be eligible for the eleventh annual Randi Perkins Scholarship the application must be turned in no later than May 1, 2014. The amount of the scholarship is $1500. The scholarship committee will review all applications and notification of the recipient will be sent out sometime between May 10th and May 15th. It is my hope that this scholarship will assist you in pursuing your dreams and goals as you leave your high school years behind. I am very proud to be able to award this scholarship for Randi. Sincerely, Teresa Lewis Randi’s Mom Scholarship Application Qualifications: 1. The applicant must be a 2013 female graduate from the St. Louis Metro Area 2. She must be a female athlete as recognized by her high school or other sources of recommendations. 3. To apply for the scholarship the student must submit the completed application, two letters of recommendation, and the completed essay questions. 4. Show proof of enrollment to an institution of higher education before the scholarship will be awarded. 5. The deadline for submission of application for consideration is May 1, 2013 Application Instruction 1. Student should attach personal Resume 2. Attached to the Resume should be all letters of recommendations and required essays. 3. Student should notify her school counselor and any coaches that have any affiliation. Their names and phone numbers should be included on the resume. 4. Resume should include any additional information about their involvement in any other sports/activities outside of high school. 5. Explain the role women’s sports have played in your life with emphasis on how your participation in high high school sports has influenced your outlook on life in general. 2. For your senior service project, you have agreed to be a mentor at a summer camp for three weeks. You have been informed that your student is a sixteen year old female that is diagnosed with a rare form of cancer. What do you do to prepare for the three week project? Include any resources you might consult, concerns, and feelings you have about your student.
